SAN JOAQUIN <br />-:aU; i y <br />Environmental Health Department <br />AMENDED <br />1/21/2020 <br />Large Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />PEP BOYS #711 <br />4987 WEST LN, STOCKTON <br />January 13, 2020 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />119 <br />HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br />OBSERVATION: Each of the following World Oil consolidated manifest receipts found on-site, each for the <br />disposal of 1 drum of non-RCRA hazardous waste used metal oil filters containing oil, were missing the manifest <br />number and the identification number of the authorized facility to which the hazardous waste was transported: <br />- Service Order No. N3082435 signed by the generator on 11/27/2019 (see photo IMG 0397) <br />- Service Order No. N3058678 signed by the generator on 10/29/19 (IMG 0398) <br />- A service order signed by the generator on 7/15/19 (IMG 0399) <br />- Service Order No. N2697915 signed by the generator on 10/17/18 (IMG 0401) <br />- Service Order No. N2670374 signed by the generator on 9/17/18 (IMG 0402) <br />- Service Order No. N2570748 signed by the generator on 5/21/18 (IMG 0403) <br />REGULATION GUIDANCE: (4) All copies of each receipt shall contain all of the following information: <br />(A) The name, address, identification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator's representative. <br />(B) The date of the shipment. <br />(C) The manifest number. <br />(D) The volume or quantity of each waste stream received, its California and RCRA waste codes, the wastestream <br />type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br />Nations/North America (UN/NA) identification number, if applicable. <br />(E) The name, address, and identification number of the authorized facility to which the hazardous waste will be <br />transported. <br />(F) The transporter's name, address, and identification number. <br />(G) The driver's signature. <br />(H) A statement, signed by the generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />CORRECTIVE ACTION: Provide a corrective action statement and supporting documentation to the San Joaquin <br />County Environmental Health Department within 30 days. <br />This is a Class II violation. <br />FA0003969 PR0513935 SCO01 01/13/2020 <br />EHD 22-02 Rev. 9/20/2019 Page 9 of 19 Large Quantity Hazardous Waste Generator OIR <br />1868 E.
