;ONDITIONS OF APPROVAL ATTACHMENT A <br /> QX-86-5 <br /> Frank & Helen Alegre <br /> All conditions shall be met prior to the commencement of opera- <br /> tions. <br /> 1 , San Joaquin Local Health District : <br /> a. No excavation may be permitted into the ground water table <br /> without approval from the Central Valley Regional Water <br /> Quality Control Board . <br /> b. The materials from an excavation shall not be deposited <br /> into any water course nor in any way contribute to the <br /> pollution thereof . <br /> c . Provide an approved potable water supply for all employees . <br /> d. Approved chemical toilets shall .be provided for all <br /> employees . Toilets must be maintained in a clean and <br /> sanitary manner . <br /> e . Handwashing facilities. must be provided on or near the <br /> approved chemical toilets . <br /> f . The ash stockpile is to be covered or under a roof and <br /> stored on an impermeable pad with run-off control . <br /> g. Test results of the ash is to be furnished to the Local <br /> Health District on a periodic basis (quarterly, and change <br /> of coal supply) . <br /> h. The ash is to be handled and stored so that it will not <br /> create a threat to water quality. <br /> i . Waste water from the storage area must be collected, and <br /> it can only be used to wet the storage pile . <br /> 2 . Depart gent of Public Works : <br /> a. Prior to the commencement of operations , the E:;cavaticn/ <br /> Rehabilitation site plan shall be revised to include the <br /> following and submitted for approval of the Department of <br /> Public Works : <br /> ( 1 ) Provide a detail of the access road from the public <br /> road. The first 100 feet of any access road shall be <br /> surfaced in a manner acceptable by the County . <br /> ( 2 ) Provide a detail of the access road at the Union <br /> Pacific Railroad crossing, and provide evidence of a <br /> crossing permit from the railroad . <br /> ( 3 ) A new road alignment shall be constructed and in use <br /> within three months of the issuance of the permit . <br /> The alignment is to be located at the railroad under- <br /> crossing road that borders the west end of the parcel . <br />
