Ladders shall <br /> be placed at an angle not more than 30 degrees from <br /> vertical and secured as necessary. Ladder side rails <br /> shall extend at least three feet above the original <br /> ground surface. <br /> 4.9.5 Confined Space Entry <br /> Entry into confined spaces is not anticipated. However, if it is <br /> necessary. entry will be performed in accordance with all <br /> applicable federal and state regulations. Steps that will be <br /> followed prior to employee entry into a confined space include: <br /> Employee training and indoctrination of confined space <br /> entry procedures prior to performing any such work. <br /> Initial hazard assessment will be completed that <br /> includes atmospheric testing of the confined space for, <br /> r at a minimum, oxygen content, flammability, and toxic <br /> contaminants. <br /> Mechanical ventilation, as appropriate, will be <br /> provided. <br /> The "Inspections and Tests for Entry Into Confined <br /> Spaces" form must be completed and posted prior to <br /> entering the space. <br /> Supplied air respirators shall be required for confined <br /> space entry if preentry atmospheric testing indicates <br /> contaminant concentrations greater than two times the <br /> TLV. <br /> Entry shall be made while wearing a lifeline and <br /> harness that is manned by a standby person. <br /> 4.10 SANITATION <br /> 4-27 <br />
