This physician will be <br /> familiar with the effects of the chemicals on site and will <br /> be used as a consultant in the event of a job-related <br /> .illness. <br /> If an injury or illness is the result of a chemical <br /> exposure, a super-visor shall promptly initiate the steps <br /> necessary to identify the chemical(s) . Chemical <br /> identification shall be accomplished through use of <br /> monitoring equipment [photoionization detectors (Pigs) in <br /> conjunction with detector tubes and/or conventional <br /> industrial hygiene monitoring] and any prior sampling <br /> results that are available. Such information shall be made <br /> 1 available to the treating physician and the H&S Coordinator. <br /> Any injury or illness requires the completion of IT Form <br /> 9300.1.-1, "Supervisor's Employee Injury Report", in <br /> accordance with IT Procedure 9300.1.. <br /> Any injury/illness not limited to a first-aid response <br /> requires that the Field supervisor immediately notify the <br /> H&S Coordinator. This notification allows the coordination <br /> of internal resources to assist the treating physician in <br /> rendering appropriate care. <br /> Any employee of IT or of a subcontractor who is suspected of <br /> having an overexposure to the chemicals on site will be <br /> given another complete physical examination. Any employee <br /> or contractor who develops a lost-time illness or sustains a <br /> 4-9 <br />
