' 11.3 Medical Requirements <br /> Hepatitis B Vaccination <br /> All potentially exposed employees will have made available to them at no cost a Hepatitis B <br /> vaccination. The employee will also receive training as to the vaccine's efficacy, safety, <br /> benefits, and consequences prior to administration. The vaccination series shall be initiated <br /> within 24 hours of providing first aid/CPR in an incident and shall be administered under the <br /> supervision of a licensed physician. Employees may at their own discretion decline the <br /> vaccination, in which case documentation of declination will be completed and employees <br /> may be assigned immediately. If an employee covered by this exposure plan decides to <br /> accept the vaccination at a later date, the vaccination will be offered at that time at no cost to <br />' the employee- <br /> Post-Exposure Procedures and Evaluation <br /> Subsequent to all reported exposure incidents, a confidential medical evaluation and follow-up <br /> shall be made available to each employee exposed in the incidents. <br /> Documentation Procedures <br /> Documentation of the exposure incident shall be recorded as soon as possible, and include the <br /> route(s) of exposure, the circumstances surrounding the incident, and the identification of the <br /> source individual. Additionally, each incident shall be placed on the "first aid incident list" <br /> attached to the location OSHA Log of Occupational Injuries and Illnesses. <br /> Blood Testing <br /> As soon as feasible, the source individual in an exposure incident will be asked to consent'to <br /> a blood test to determine HBV and HIV infectivity. Where applicable laws require employee <br /> consent, documented consent shall be obtained prior to testing. If an employee refuses the <br /> blood test, documentation of the refusal will be made. Documentation of the test results shall <br /> be made available to the exposed employee(s). All results should be kept confidential, as <br /> criminal and civil penalties may be charged against persons negligently or wilfully releasing <br /> such information, depending on local laws. <br /> wa5-09-94M5 tA-00" 11-5 <br />
