Exposed employees will be asked to consent to a blood test for HBV and HIV serological <br /> status. If consent to HIV testing is denied, the blood sample will be preserved for 90 days, <br /> within such time the employee may elect to consent to the HIV test. <br /> Post Exposure Medical Evaluations <br /> Exposed employees shall receive a healthcare professional's written opinion for post-exposure <br /> evaluations. The written opinion shall include the results of the evaluation and any medical <br /> conditions resulting from the exposure incident which requires further medical treatment. <br /> 11.4 Hazard Communication <br /> • Containers used for disposal of blood contaminated supplies and waste will be <br /> labeled in accordance with the word "biohazard." <br /> • Warning signs are not applicable, as there are no designated areas for medical <br /> treatment on site. In cases of potential exposure observers and non essential <br /> personnel should be verbally warned to keep a safe distance from injured <br /> personnel. <br /> All associates who are first aid/CPR trained and may provide assistance shall be <br /> trained in the requirements of HS512 and this HASP. <br /> 11.5 Recordkeeping <br /> Training Records <br /> All employees on the project shall review this plan and sign it to document their review. In <br /> addition, all employees who attend Bloodborne Pathogen training shall sign the class Training <br /> Attendance Form. The training record will contain the date; training outline; name and <br /> qualifications of the trainer, and names and job titles of attendees. All participants must take <br /> and pass the training quiz. The training records will be maintained by the IT Training <br /> Department for at least three years from the training date. <br /> Medical Records <br /> Medical records necessary for IT employees must include documentation on HBV vaccination <br /> status, medical follow-up, post-exposure testing, and a medical professional's written <br /> evaluation.
