SAN JOAQUIN; STOCKTON <br /> AEMC JOB NO. 80540 <br /> An incomplete proposal for further assessment and/or remediation of the <br /> contamination documented at the above referenced site has been submitted to this <br /> office. Enclosed is an informational packet outlining the steps to obtain the <br /> necessary permits to facilitate the investigation. <br /> Pursuant to the San Joaquin Local Health District fee schedule Resolution No. 85-581 <br /> a $35.00 consultation fee shall be submitted along:, with the completed general <br /> environmental health application form for review of the above referenced <br /> environmental assessment/remediation activity. <br /> Each additional hour over the initial one hour of time spent in review activity <br /> will be billed at the rate of $35.00 per hour. The additional amount due will be <br /> billed to the applicant identified on the general environmental health application <br /> form at the completion of requested activity, or monthly, if longer than 30 days is <br /> required for completion. <br /> The current workload volume is being managed by a 60-day backlog of chronologically <br /> received environmental assessment/remediation reports and/or workplans. The San <br /> Joaquin Local Health District has established a priority review process which is an <br /> option available upon written request. The priority review is conducted during off <br /> hours at an overtime rate of $52.50 an hour. Scheduling of the priority review is <br /> accomplished by individual staff members, but can generally occur within 10 working <br /> days of the receipt of written request and the submitted assessment document. <br /> Should you have questions concerning this matter, contact Diane M.
