POST AT FACILITY <br /> EMERGENCY RESPONSE NOTIFICATION PROCEDURE <br /> FOR MAJOR GAS SPILL <br /> • TURN OFF PUMPS: Using the Emergency Pump Shut-Off Switch. <br /> • EVACUATE: verbally ANNOUNCE to all persons on the site "This is an emergency! Please turn off all your <br /> engines and leave the station on foot immediately." All employees meet at the emergency assemble area. <br /> • CALL 911: Give the following information: NATURE OF EMERGENCY AND SITE ADDRESS. If anyone is trapped <br /> or in need of medical attention;tell the answering dispatcher. Stay on the phone and be prepared to answer <br /> any other questions concerning the emergency. <br /> • ATTEMPT: To contain the spill if you can do so safely. Clean up with absorbent materials (dry method) on <br /> site and dispose of in the 55-gallon hazardous waste drum on site. Have a fire extinguisher ready for spills or <br /> flammable materials. <br /> • LOOK AROUND: To ensure that everyone has left the station, particularly those in vehicles who may need <br /> assistance or may not have heard the emergency announcement. Assist or direct assistance to anyone having <br /> difficulty leaving the station and anyone who may be injured. <br /> • REPORT: To arriving emergency response personnel and provide them with any information or assistance <br /> they may require. <br /> • NOTIFY: The following IMMEDIATELY to assist in the emergency and agency notification process: <br /> 1. Facility Management Group — (310) 808-6702 <br /> 2. 24-Hour Spill Cleanup Contractor— CURA Emergency Services — (800) 579-2872 <br /> 3. Regulatory Affairs & Compliance Manager — (562) 505-5931 <br /> • CONTACT: United Pacific's response team. Use the list below for emergency contacts. <br /> PRIMARY CONTACT SECONDARY CONTACT <br /> DISTRICT MANAGER DIVISION MANAGER <br /> Telephone Number: Telephone Number: <br /> OTHER INCIDENTS: Any incident that can be contained and cleaned up as part of routine operations. Whenever <br /> in doubt, consider the incident a MAJOR INCIDENT and use the above procedures. <br /> • FIRE: If it is safe to do so, extinguish with fire extinguisher, have fire extinguisher recharged after use. <br /> • ROBBERY: As soon as the robber leaves the store, lock the front door and call 911. Immediately notify your <br /> District Manager. Complete appropriate Incident Report. <br /> • EMPLOYEE INJURY: Call 911 if needed. Immediately notify your District Manager. Complete appropriate forms. <br /> • CUSTOMER INJURY: Call 911 if needed. Obtain customer's information and take photos of where the incident <br /> occurred.
