Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 10, 2020 <br /> Facility Address: <br /> 1721 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> KAISER PERMANENTE <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />(contents) (IMG 1260-3). <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br />words “Hazardous Waste”, name and address of generator, hazardous properties, physical state, composition <br />(contents), accumulation start date <br />CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br />action statement and supporting documentation to the San Joaquin County Environmental Health Department <br />(EHD) within 30 days. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br />finished on 4/8/2020 and replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 5/8/2020. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
