Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />March 10, 2020 <br /> Facility Address: <br /> 1721 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> KAISER PERMANENTE <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br />were: <br />- Located in the Pharmacy: <br />1. A black container with a white lid was missing the hazardous properties and the composition (contents) (see <br />photos IMG 1213-4). <br />2. A black container with a clear lid was missing the composition (contents) (IMG 1215-6). <br />3. A black container with a white lid was missing the composition (contents) (IMG 1217). <br />- Located in the Pediatrics Department: <br />4. A black container with a white lid and storing aerosols, was missing the hazardous properties (IMG 1218-20) <br />- Located in the Adult Family Medicine Soiled Utility Room: <br />5. A black container with a white lid storing silver nitrate sticks was incorrectly marked as flammable, corrosive, and <br />reactive for the hazardous properties (IMG 1221-3). According to a hazardous waste labeling guidance document, <br />posted in the room, silver nitrate sticks are to be marked as an oxidizer. <br />6. A black container with a white lid storing Cardinal Health Hot Packs and Infant Heel Warmers, was missing <br />composition (contents) (IMG 1227-8). <br />7. A black container with a white lid storing silver nitrate sticks was incorrectly marked as corrosive for the <br />hazardous properties (IMG 1229-31). According to a hazardous waste labeling guidance document, posted in the <br />room, silver nitrate sticks are to be marked as an oxidizer. <br />8. A black container with a clear lid and storing foam hand sanitizer, was missing the hazardous properties, <br />physical state, and composition (contents) (IMG 1235-7). <br />9. A black container with a clear lid was labeled as storing "expired supp" and contained several waste items, <br />including one labeled "BD Universal Viral Transport for Viruses, Chlamydiae, Mycoplasmas and Ureaplasmas." The <br />composition (contents) listed on the container as "expired supp" does not provide a clear indication as to what is <br />stored in the container. (IMG 1238-9) <br />10. A black container with a white lid and storing hot & cold compresses/packs, was missing the physical state <br />(IMG 1242-4). <br />11. A black container with a white lid and storing silver nitrate applicators, was missing the physical state and <br />hazardous properties (IMG 1245-7). <br />12. A black container with a white lid was missing the physical state, hazardous properties, and accumulation start <br />date, and was partially illegibly labeled with a composition (contents) of "cultery stick" (IMG 1248-50). <br />13. A black container with a white lid and storing expired/unopened swabs, was missing the physical state and <br />hazardous properties (IMG 1251-3). <br />14. A black container with a white lid and storing hydrogen peroxide, was missing the physical state and hazardous <br />properties (IMG 1254-5). <br />- Located in the Ultrasound area: <br />15. A black container with a white lid and labeled as storing "all test strips & container disposal," was missing the <br />physical state and hazardous properties (IMG 1256-59). <br />- Located in the Laboratory: <br />16.
