Seventh St . <br /> Lathrop, Ca. 95330 <br /> Recently your consultant, WHF, has submitted the Quarterly <br /> Monitoring Report for the third quarter-1994 . Missing from this <br /> report were the following items : <br /> - monitoring well data (pH, conductivity, water <br /> temperature, and purge volume) <br /> - laboratory data sheets showing detection limits <br /> - chain of custody form <br /> - quality control sheets (QA/QC) for water samples <br /> - DHS laboratory certification data, narrative from, and <br /> signature of, the laboratory director <br /> Until these missing items are submitted to and approved by San <br /> Joaquin County Public Health Services, Environmental Health <br /> Division (PHS-EHD) , the Quarterly Report for September 30 , 1994 <br /> (3rd quarter-1994) is invalid and cannot be accepted. <br /> In the future, please have your consultant discontinue to list <br /> the groundwater concentrations for Total Petroleum as Gasoline in <br /> the tables in mg/l (ppm) . All groundwater concentration data <br /> submitted to PHS-EHD is to be in micrograms per liter (ug/1) , <br /> also known as parts per billion (ppb) . As an additional note, any <br /> soil concentrations in future reports are to be listed in <br /> milligrams per kilogram (mg/kg) , also known as parts per million. <br /> Please have your consultant develop a schedule for quarterly <br /> monitoring well sampling that will allow adequate time for <br /> laboratory analyses, report preparation and review by yourself, <br /> and time enough to submit the report to PHS-EHD prior to the next <br /> quarterly sampling event . <br /> A Site Mitigation Schedule was included with the Quarterly Report <br /> for December 30, 1994 (fourth quarter-1994) . Activities for July <br /> 5, 1995 mention monitoring well "reestablishment" . PHS-EHD is <br /> concerned that monitoring well destructions have been mentioned <br /> but the required workplan describing the methods or materials for <br /> these proposed destructions has not been submitted. <br /> A Division of San Joaquin County Health Care Services <br />
