Such equipment must be"locked out" <br /> prior to working on it and a permit obtained from the buyer's representative. <br /> (24) Approved face masks and respirators shall be worn when handling powders,insulation or other potentially hazardous mater!als <br /> (25) Safety belts and life lines must be worn on all elevated work on scaffolds,temporary platforms,structural configuration and two- <br /> legged ladders where a barrier is absent which would prevent falls. <br /> (26) The Seiler shall not make a connection to any operating system unless authorized by the Buyer's representative. <br /> (27) The Seller's employees shall not turn off or on any of the systems mentioned in item(26).All opersting valves or switches shall be <br /> lagged out and operated by the Buyer's representative. <br /> (26) The Seller's employees shall not operate any Buyer's equipment without approval of the Buyer's representative. <br /> (29) No work shall be performed or any electrical equipment or process piping which,ss into the Buyer's existing equipment unless <br /> all electrical switches and valves are "Locked Cut" and a permit Is obtained from the Buyer's representative <br /> (30) Adequate purging and testing of all process lines is required before any work can be performed on them <br /> 4. FIRE PREVENTION <br /> (1) The Seller and his foremar shall be familiar with the location and operation of all fire fighting equipment provided for the project, <br /> (2) The procedures to be followed in case of fire, including evacuation of personnel where necessary, shall be determined by the <br /> Seiler and the Buyer's representative The Seller Shall instruct his personnel in these procedures. <br /> S. HOUSEKEEPING <br /> (1) The job site shall be kept clear and orderly at all times <br /> 6. FIRST AID AND SAFETY EQUIPMENT <br /> (1) The Seller shall provide all necessary safety equipment and enforce its use. <br /> (2) The Seller shall supply the Buyer with the names of two doctors who may be called at any time when needed.This information <br /> will be forwarded to the first aid station for reference. <br /> (3) In emergency situations first aid can be provided by the plant nurse, supervisor or guards. <br /> (4) Proper protection shall be provided the worker so that he carr perform his work safely, The Seller shall regularly inspect all <br /> safety equipment, keep ;t In good condition, and make any necessary repairs. <br /> (a) All vehicles operated by the Seller shall comply with all plant traffic rules. Previous arrangements must be made with the <br /> Buyer's security office when vehicles are to be operated on the plant site, <br /> (b) Salle's shall not operate equipment other than their own unless specific authorization Is obtained from the B:;yer. <br /> 7. PERSONAL CONDUCT <br /> (1) Seller's employees shall use only the sanitary statiors and washroom facilities designated by the Buyer <br /> (2) Running is not permitted except in case of an emergency. <br /> (3) ''Horseplay", practical jokes, or any similar activity is prohibited. <br /> (4) Seller's employees will be allowed to park personal vehicles in the Buyers parking lot.The buyer assumes no liability for toss <br /> or damage to vehicles or other personal property Please observe reserved parking areas and do not block driveways and other <br /> designated areas <br /> (5) Some areas of the plant have restricted access Do not enter any area other then your assigned work area without prior approval <br /> of the Buyer's representative. <br /> 6. VEHICLES <br /> Authorized company vehicles displaying logos or identified to Security as a company vehicle will be permitted access to the main plant. <br /> All other vehicles will be parked In the main parking tot. No private vehicles are permitted In the plant without security authorization. <br /> 9. CAMERAS <br /> The use of cameras, other photographic, electronic or recording equipment must be authorized by the Security Dept only <br /> 3!1/83 <br /> Form S-54 <br />
