R . Simplot Company facility in Lathrop, California, all workers <br /> brought into the facility by contractors are required to sign this form <br /> to assure Simplot management that they have been informed of the basic <br /> plant safety rules and to assure that their employer has provided them <br /> with appropriate safety training for the duties that they are to <br /> perform . <br /> 1 . All workers are to wear hard hats and glasses with side shields , long <br /> (Dr short sleeved shirts , trousers and hard soled shoes at all times when <br /> in the facility . <br /> 2 . Workers doing above ground work where safety ladders and platforms <br /> are not available will wear a safety belt when performing work where a <br /> fall of 15 feet or more is possible . <br /> 3 . When electrical or mechanical work is being done on equipment that is p <br /> power driven , the equipment is to be shut off at the motor control <br /> center breaker and locked with a lock out padlock. <br /> 4 . When work is to be performed in or on piping or vessels, a work <br /> permit is to be obtained from the plant operating department . <br /> 5 . When work is to be performed using cutting or welding equipment , a <br /> work. permit Is to be obtained from the operating department . j <br /> 5 . Smoking is not permitted in any facility building. <br /> I have been informed of the above basic plant safety rules and I have <br /> had specific training by my employer on safety procedures for performing <br /> my specific type of work specialty . <br /> Company Name <br /> Signature _ bate <br /> i <br /> workers not following these procedures will be asked to go to the <br /> technical service department for special training. After receiving this <br /> training , they will by asked to sign a copy of this document . <br /> A back charge of $50 will be made to the contractor for training costs <br /> if a signed copy of this document is not in the workers possession or in <br /> the hands of the technical service manager .
