MONITORING AND REPORTING PROGRAM NO. 97-229 - 12 - <br /> J.R. <br /> 12 -J.R. SIMPLOT LATHROP FACILITY <br /> SAN JOAQUIN COUNTY <br /> A letter transmitting the self-monitoring reports shall accompany each report. Such a letter shall <br /> include a discussion of requirement violations, such as operation or facility modifications. If the <br /> Discharger has previously submitted a report describing corrective actions and/or a time <br /> schedule for implementing the corrective actions, reference to the previous correspondenceill <br /> be satisfactory. The transmittal letter shall contain a statement by the Discharger, or the <br /> Discharter's authorized agent, under penalty of perjury, that to the best of the signer's <br /> knowledge the report is true, accurate, and complete. <br /> Revision of this monitoring program, including changes to monitored parameters and/or <br /> monitoring frequencies outlined herein, will be considered pending submission of specific <br /> justification and rationale. <br /> The Discharger shall implement the above monitoring program on the effective date of this <br /> Order. <br /> Facility Monitoring Report <br /> 25. By 15 November each year, the Discharger shall submit an annual report describing the <br /> results of the inspection and the repair measures implemented, including photographs of <br /> the problems and the repairs. <br /> 26. The results of any monitoring done more frequently than required at the locations specified <br /> in the MRP also shall be reported to the Regional Water Board.
