& SAFET AND ENVIRONMENTAL Fat ES FOR <br /> Simplot <br /> SUPPLIERS ON SIMPLOT PREMISES <br /> 1. INTRODUCTION <br /> In the interest of brevity,the following definitions will be used throughout this booklet <br /> BUYER:J.R.Simplot Company(Simplot) <br /> SELLER:Any corporation,partnership,individual,joint venture,firm or company that performs work on J.R.Simplot's premises. <br /> This directive sets forth the safety rules and procedures which must be followed by the Seller.Sellers are also responsible for providing their subcontractors with a copy of this directive <br /> and seeing that such subcontractors and their employees comply with these rules and procedures. <br /> Sellers engaged in construction or services for the Buyer are fully responsible for the safe performance of all work for which they are contractually committed,whether done by <br /> themselves or by their subcontractors.THE SELLER MUST PROVIDEADEOUATE PROTECTION FORTHE HEALTH AND LIFE OFTHE BUYER'S EMPLOYEESAND THE GENERAL <br /> PUBLIC,AS WELL AS FOR HIS OWN PERSONNEL.HE MUST COMPLY WITH ALL PROVISIONS OF THE LATEST LOCAL,STATE AND FEDERAL REGULATIONS. <br /> Some of the safety rules which are listed in this directive are similar to focal,state and federal regulations,but are repeated for emphasis.Others are required because of the special <br /> nature of the Buyers equipment,and have evolved from many years of experience. <br /> Naturally,the safety rules given here cannot cover every eventuality.It is expected,therefore,that the seller will exercise good judgement in all safety matters,even though not <br /> specifically mentioned.It is further required that the Seller work closely with the Buyer's Representative in resolving all safety problems. <br /> 2. PURPOSE <br /> The primary purpose of this directive is to eliminate personnel injury and equipment damage by preventing accidents.Unauthorized personnel shall be kept off the premises to eliminate <br /> undue congestion and prevent the possibility of their being involved in an accident.Any admittance of union representative or governmental agencies must have prior approval of the <br /> Buyer. <br /> 3. Where the Sellers employees will be involved in working on major construction projects the Seller must provide the following documentation to the Buyers representative prior to <br /> performing any work on the Buyers premises. <br /> (a) A copy of the Sellers Injury,Injury Prevention Plan. <br /> (b) Copies of the Sellers OSHA 200 Injury Log and site safety inspections tions covering the most recent contract period with the Buyer. <br /> (c) Copies of appropriate environmental permits required for operation of any of Seder's equipment to be used on Buyer's property. <br /> Where the Sellers employees will be involved in working on or near systems that process or convey acutely hazardous materials the Seller must also provide the following documentation <br /> to the Buyers representative prior to performing any work on the Buyers premises. <br /> (a) Copies of Sellers records of employee training in safety procedures which shall have recorded the Name of the employee,date of training, <br /> and the method used to evaluate the effectiveness. <br /> The training must cover the following areas as applicable to the work to be performed: <br /> (1) Respiratory protection program(including fit testing records). <br /> (2) Selection and use of personal protection equipment. <br /> (3) Crane and Industrial Lift Truck operator training. <br /> (4) Emergency procedures. <br /> (5) Hazards Communication training(Right to Know/MSDS Law). <br /> 4. The Buyer will provide the Seller with the following information to be presented to the Sellers employees.The Seller must document that this information was presented to the Sellers <br /> employees and provide the Buyer with copies of said documentation. <br /> (a) Provisions of the Buyers Emergency Action Plan. <br /> (b) Employee evacuation routes. <br /> (c) Summary of Hazards associated with the process the Sellers employees will be involved with. <br /> (d) Buyers location and contact person for obtaining Material Safety Data Sheets(MSDS), <br /> The Sellers employees will be required to read and sign a summary of the Buyers Contractors Safety Procedures before starting any work on the Buyers premises. <br /> 5. GENERAL SAFETY REQUIREMENTS <br /> (1) The Seller must follow the safe practice procedures for all work performed.He will be expected to operate within the regulations at all times and take the necessary steps to <br /> eliminate unsafe working conditions. <br /> (2) The Seller shall comply with all state,local and public safety codes,laws or ordinances pertaining to the work being performed. <br /> (3) BEFORE STARTING THE WORK SPECIFIED IN A CONTRACT,THE SELLER SHALL MEET WITH THE BUYER'S REPRESENTATIVE FOR A BRIEFING ON THE BUYER'S <br /> SAFETY STANDARDS FOR THE PARTICULAR JOB. <br /> (4) The Seller shall report all injuries sustained by his personnel during the contract period to his insurance carrier.Two(2)copies of this report shall also be sent to the Buyer.Reports <br /> of all accidents shall be submitted in writing within twenty-four hours. <br /> (5) As required by Workmen's Compensation laws,the Seller must have the required insurance coverage and is responsible for reporting his employee's occupational injuries to the <br /> proper authorities.The Seller shall not release any accident information to the press.All inquiries on accidents should be referred to the Buyer for handling. <br /> (6) Instructions(printed or typewritten)are to be strategically posted by the Seller on the procedure to be followed in obtaining emergency help,e.g.,doctor, ambulance,fire <br /> department,and police. <br /> (7) Suitable hard hats must be wom at all times by all personnel working at or visiting the job site.Hard hats for the Seller's personnel shall be supplied by the Seller. <br /> (8) Safety glasses with side shields suitable for the work being done must be wom by personnel working at or visiting the job site. <br /> (9) The wearing of safety shoes by all personnel working on the site shall be encouraged. <br /> (10) Hearing protecficn will be wom when conditions warrant. <br /> (11) Smoking is permitted ONLY in areas designated by the Buyer. <br /> (12) Under no circumstances shall alcoholic beverages be permitted on the property.Persons under the influence of alcohol or drugs must be immediately removed from the premises. <br /> Firearms are prohibited on the property. <br /> (13) The Seller shall conduct weekly safety meetings with his personnel. <br /> (14) The Seller's foremen shall explain the safety requirements of the job to each of the men for whom he is responsible.They shall emphasize the job safety requirements at all times. <br />
