(15) The Seller and subcontraCor shall immec ;orrect any unsafe working condition which may arise. <br /> (16) Special written approval of Buyer's repress—.,ve is required before work of the following types is performed. A i,—.iit must be obtained and approved by Seller'representative. <br /> (a) Where there is a passibility of oxygen enriched or oxygen deficient atmospheres:acceptable limits are 20 to 22 percent oxygen. <br /> (b) Where toxic agents are used or are present. <br /> (c) Pressure testing. <br /> (d) Work inside vessels or other enclosures or pits. <br /> (e) Work on electrical circuits over 110V. <br /> (f) Torch work on equipment where flammable gases or liquids could be involved. <br /> (g) In areas or on equipment and on pipe lines which contain hazardous materials. <br /> (17) Suitable and appropriate clothing must be worn while engaged in legitimate company business within the plant perimeters.Short sleeved shirts are permissible with shoes and full <br /> length trousers.Tank tops,tennis shoes,open toed shoes,shorts,and no shirts worn are not in compliance with the company safety policy.Appropriate protective clothing must <br /> be worn when working with hazardous materials or other operations which could cause injury to the lungs,skin,eyes or other body parts, <br /> (18) While work is being performed inside a confined space,at least one designated person shall be immediately available to secure or render assistance in case of an emergency. <br /> (19) A safety harness with a life line shall be wom by a worker whenever he is required to enter a confined space.Exceptions to this rule must be approved by the Buyer in writing on <br /> the permit <br /> (20) No pipelines or vessels shall be opened under pressure. <br /> (21) Adequate scaffolding and ladders shall be provided where required.Erection,use,and type shall conform to all applicable regulations. <br /> (22) All flammable gas cylinders,high pressure cylinders,and similar containers shall be stored in property identified and designated areas. <br /> (23) No work shall be performed on electrical equipment that is energized(110V or above).Such equipment must be'locked out'prior to working on it and a permit obtained from the <br /> buyer's representative. <br /> (24) Approved face masks and respirators shall be worn when handling powders,insulation or other potentially hazardous materials. <br /> (25) Safety harnesses and rife lines must be worn on all elevated work on scaffolds,temporary platforms,structural configuration where a barrier is absent which would prevent falls. <br /> (26) The Seller shall not make a connection to any operating system unless authorized by the Buyer's representative. <br /> (27) The Seller's employees shall not tum off or on any of the systems mentioned in item(26).All operating valves or switches shall be tagged out and operated by the Buyers <br /> representative. <br /> (28). The Seller's employees shall not operate any Buyer's equipment without approval of the Buyer's representative. <br /> (29) No work shall be performed on any electrical equipment or process piping which ties into the Buyeds existing equipment unless all electrical switches and valves are'Locked Out' <br /> and a permit is obtained from the Buyer's representative. <br /> (30) Adequate purging and testing of all process lines is required before any work can be performed on them. <br /> (31) Seiler will have available a current Material Safety Data Sheet(MSDS)for any hazardous materials brought on to the Buyer's site for use by the Seller or Seller's employees. <br /> (32) Any hazardous material brought on to the Buyer's site for use by the Seller or Seller's employees shall be property used and stored. <br /> 6. FIRE PREVENTION <br /> (1) The Seiler and his foreman shall be familiar with the location and operation of all fire fighting equipment provided for the project <br /> (2) The procedures to be followed in case of fire,including evacuation of personnel when:necessary,shall be determined by the Seller and the Buyer's representative.The Seller shall <br /> instruct his personnel in these procedures. <br /> 7. HOUSEKEEPING y ENVIRONMENTAL <br /> (1) The job site shall be kept clean and orderly at all times. <br /> (2) The Seller or his employees shall not dispose of any materials on the Buyer's property without prior consent of Buyer's representative. <br /> (3) All excess materials brought on to the job site by Seller will be removed by the Seller at the end of the job unless prior arrangements have been made with the Buyer's <br /> representative. <br /> & FIRST AID AND SAFETY EQUIPMENT <br /> (1) The Seller shall provide all necessary safety equipment and enforce its use. <br /> (2) The Seller shall supply the Buyer with the names of two doctors who may be called at any time when needed.This information will be forwarded to the first aid station for reference. <br /> (3) In emergency situations first aid can be provided by the plant nurse,supervisor or guards. <br /> (4) Proper protection shall be provided the worker so that he can perform his work safely.The Seller shall regularly inspect all safety equipment,keep it in good condition,and make <br /> any necessary repairs. <br /> (a) All vehicles operated by the Seller shall comply with all plant traffic rules.Previous arrangements must be made with the Buyer's security office when vehicles are to be <br /> operated on the plant site. <br /> (b) Sellers shall not operate equipment other than their own unless specific authorization is obtained from the Buyer. <br /> 9. PERSONAL CONDUCT <br /> (1) Seller's employees shall use only the sanitary stations and washroom facilities designated by the Buyer. <br /> (2) Running is not permitted except in case of an emergency. <br /> (3) 'Horseplay',practical jokes,or any similar activity is prohibited. <br /> (4) Seller's employees will be allowed to park personal vehicles in the Buyer's parking lot.The buyer assumes no liability for loss or damage to vehicles or other personal property. <br /> Please observe reserved parking areas and do not block driveways and other designated areas. <br /> (5) Some areas of the plant have restricted access.Do not enter any area other than your assigned work area without prior approval of the Buyer's representative. <br /> 10. VEHICLES <br /> Authorized company vehicles displaying logos or identified to Security as a company vehicle will be permitted access to the main plant.All other vehicles will be parked in the main <br /> parking lot.No private vehicles are permitted in the plant without security authorization. <br /> 11. CAMERAS <br /> The use of cameras,other photographic,electronic or recording equipment must be authorized by the Security Department only. <br /> S-54(PMSBAFETY 2/95) <br />
