APPLICATION FOR PERMIT TO CONNECT PRIVATE TILE DRAIN TO NSW JERUSALEM <br /> DRAINAGE DISTRICT DRAIN FACILITIES . <br /> To The Board of Directors of the New Jerusalem Drainage District <br /> The undersigned desires to connect an do-farm tile drainage <br /> system to the collector lanes of the New Jerusalem Drainage District . <br /> Attached to this application is a drawing prepared by the U. S . D .A. <br /> Soil Conservation Service showing the location , type of connection <br /> and other data pertinent to the proposed connection . <br /> in making this application I understand and agree to all of the <br /> following; <br /> (a) A11 work shall be done is accordance with the Construction <br /> Requirements `or Tie-In to New srerusalem Drainage District P4pelines <br /> and the accompanying drawings whiah are. attached to this appiication <br /> and incorporated herein by reference . <br /> (b) All conetruction is to be at my sole expense and at no cost <br /> to the district. <br /> (c) I agree to pxomptly repair or replace all damage to the New <br /> Jerusalem collector pipeline system caused by the activities of myself <br /> or may agents , contractors or employees in making the requested <br /> connncti.on . <br /> (d) Nothing save underground :subsurface drainage water shall be <br /> permitted to flow into the collector system of the New Jerusalem <br /> Drainage District and I specifically agree that no domestic sewage , <br /> irrigation tail water or other surface water or any other substances <br /> shall be permitted to enter the district ' s collector system ` om my <br /> lines _ <br /> (e) 1 auree to save and hojd harmless the New Jerusalem Drainage <br />
