A bottle of Cascade Complete Gel Dishwasher Detergent, which had a piece of its cap missing. <br /> h. A container of Gain liquid laundry detergent,which had its label partially missing. <br /> 2. Violation 106— <br /> a. Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> i. Oscar Arvayo, Store Manager, stated that if the hand-held scanner does not indicate which HW <br /> tote an item belongs in, employees refer to the Dollar General (DG) Environmental Compliance <br /> poster(Poster) and guidance documents found posted next to the HW storage area, as well as <br /> the labels on the hazardous waste totes. Oscar stated that if he is still not able to determine if a <br /> waste item is hazardous or which hazardous waste tote the waste item should be placed in, he <br /> will contact the Loss Prevention manager. Oscar was not aware of the appropriate person or <br /> phone number to call in case he or another employee had questions about an item's hazardous <br /> waste determination. <br /> b. Submit proof of completion of 2019 California Environmental Compliance for Store Managers training <br /> for Adri Delsied. <br /> 3. Violations 110& 111—Submit manifest 010788838FLE (12/7/17) containing the alternate facility signature.The <br /> copy submitted does not contain the signature. <br /> Dollar General#13593: <br /> 1. Violation 106— <br /> a. Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> i. Two containers of DG auto SAE 10W-40 High Mileage Motor Oil, one of which was leaking,were <br /> found in a tote.The oil was leaking out of the bag and into the bottom of the tote. It was unclear <br /> if the motor oil belonged in the Toxic or the Flammable HW tote, since the DG Poster states that <br /> automotive fluids are classified as toxic, while oils, hydrocarbons and petroleum distillates are <br /> classified as flammable. Christyne Cahill, Manager On Duty, cleaned up the leakage of motor oil <br /> from the bottom of the container, double bagged the leaking containers and clean-up material, <br /> and placed the bag into the Flammable HW tote. Christyne did not seem aware of who to call in <br /> case she or another employee had further questions about an item's hazardous waste <br /> determination. <br /> b. Submit proof of completion of 2019 California Environmental Compliance for Sales Associates training <br /> for Steven Takehana and Jay Montantes <br /> 2. Submit a statement documenting how the facility manages expired nicotine cessation items. At the time of <br /> inspection,the facility did not have a hazardous waste container designated for waste nicotine/smoking <br /> cessation items in the hazardous waste storage area, as shown in the Dollar General Environmental Compliance <br /> Program Training. Christyne Cahill, Manager On Duty, stated that expired tobacco items, including expired <br /> nicotine cessation items, were recently mailed from the facility to a vendor, Nash Finch Bellefontaine.After <br /> further discussion with John Salvino, Store Manager, it was unclear if the facility did in fact mail expired nicotine <br /> cessation items to Nash Finch Bellefontaine. <br /> Dollar General#13592: <br /> 1.
