a. Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> i. Jeffrey Delgado,Assistant Store Manager, was not aware of who or the phone number to call <br /> when he has questions about a waste item's hazardous waste determination.Jeffrey was not <br /> aware of the phone number to call to access item Safety Data Sheets (SDS), which was found on <br /> the Dollar General Safety Information Center poster in the employee break room. <br /> ii. When asked how employees clean up chemical spills and manage the waste generated from <br /> those spills,Jeffrey Delgado,Assistant Store Manager, stated spill magic is used to absorb a <br /> liquid chemical spill, which is then swept up and thrown into the trash. <br /> b. Submit proof of completion of 2019 California Environmental Compliance for Sales Associates training <br /> for Bre'LI Goodlow and Claudia De La Cruz. <br /> 2. Violation 108—Submit a corrective action statement describing how the generator has ensured that all <br /> manifests are properly completed for transport of a hazardous waste, specifically addressing the issue of the <br /> 2018 and 2019 uniform hazardous waste manifests being completed using an incorrect generator EPA ID <br /> number, CAR000278318. <br /> Dollar General#14130: <br /> 4. Violation 103-Submit documentation that demonstrates to employees which hazardous waste category(i.e. <br /> Toxic, Flammable, Alkaline,Acid, Oxidizer, etc.)the following wastes belong to: <br /> a. Glade candle (found in a Toxic Hazardous Waste tote) <br /> b. Pine Glo Antibacterial and Disinfectant cleaner(found in a Toxic Hazardous Waste tote) <br /> 2. Violation 106—Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> a. When asked how employee's determine which HW tote a waste item should be placed into, Bill Davis, <br /> Store Manager, did not know who to call if he had questions. Bill was not aware to call the phone <br /> number for M.S.D.S, listed on the Spill Response Tips poster posted next to the HW storage area, in <br /> order to obtain more information about hazardous material and item Safety Data Sheets (SDS). <br /> 3. Violation 405—Submit a corrective action statement describing specifically how the incompatible waste <br /> storage, listed on the report, has been changed to correct the violation.
