3. Submit a statement and supporting documentation explaining how the following waste was managed: un <br /> unlabeled container of unknown liquid, which appeared to be used motor oil, was found on the ground outside <br /> of the Dollar General dumpster enclosure. Steve Bohnak, District Manager, indicated the container was likely left <br /> there illegally by an unknown individual. <br /> Dollar General#14353: <br /> 1. Violation 103-Submit documentation that demonstrates to employees which hazardous waste category(i.e. <br /> Toxic, Flammable, Alkaline,Acid, Oxidizer, etc.)the following wastes belong to: <br /> a. A bag of Hickory Charcoal Briquets <br /> b. A bag of six damaged Citrus Zest candles <br /> c. DG Beauty Eye make-up Remover Pads <br /> d. An L.A. Colors Lash Adhesive <br /> e. Super Glue and Gorilla Glue <br /> f. A bottle of Gain In-Wash Scent Booster and two bottles of Downy In-Wash Scent Beads <br /> 2. Violation 106—Submit proof of completion of 2019 California Environmental Compliance for Sales Associates <br /> training for Shacala Harlin, Reshaud Kidd, Danielle Hunt, Gloria Villarreal, Gerard Rangel, and Jonathan Baugh. <br /> 3. Violation 405—Submit a corrective action statement describing specifically how the incompatible waste <br /> storage, listed on the report, has been changed to correct the violation. (Refer to inspection report violation 405 <br /> for a detailed explanation of the violation). <br /> Dollar General#13632: <br /> 1. Violation 102-Submit documentation that demonstrates to employees which hazardous waste category (i.e. <br /> Toxic, Flammable, Alkaline,Acid, Oxidizer, etc.)the following wastes belong to: <br /> a. A container of Clorox disinfectant wipes. <br /> b. A container of Top Job laundry bleach. <br /> c. A container of Tide Simply Odor Rescue laundry detergent. <br /> d. A bag of Hickory Charcoal Briquets, which was torn open. <br /> e. A bottle of DG Home Fabric Refresher,which had a broken spray nozzle. <br /> f. Two packs of Studio Selection makeup removing facial towelettes (value twin pack). <br /> g. A bottle of Cascade Complete Gel Dishwasher Detergent, which had a piece of its cap missing. <br /> h. A container of Gain liquid laundry detergent,which had its label partially missing. <br /> 2. Violation 106— <br /> a. Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> i. Oscar Arvayo, Store Manager, stated that if the hand-held scanner does not indicate which HW <br /> tote an item belongs in, employees refer to the Dollar General (DG) Environmental Compliance <br /> poster(Poster) and guidance documents found posted next to the HW storage area, as well as <br /> the labels on the hazardous waste totes. Oscar stated that if he is still not able to determine if a <br /> waste item is hazardous or which hazardous waste tote the waste item should be placed in, he <br /> will contact the Loss Prevention manager. Oscar was not aware of the appropriate person or <br /> phone number to call in case he or another employee had questions about an item's hazardous <br /> waste determination. <br /> b. Submit proof of completion of 2019 California Environmental Compliance for Store Managers training <br /> for Adri Delsied. <br /> 3. Violations 110& 111—Submit manifest 010788838FLE (12/7/17) containing the alternate facility signature.The <br /> copy submitted does not contain the signature. <br /> 5 <br />
