Brinderson will hold <br /> a daily tailgate meeting. Each tailgate will include a description of each day' s work, a list <br /> of possible hazards, steps to mitigate hazards, go over all emergency scenarios, prepare a <br /> listof workers each day to be presented to the Terminal Operator, and obtain the required <br /> permits for that day' s workrip or ;to starting work each day. dote Permit Requester <br /> (Br-inderson-person-requesting-permit) will-be-on site-'at 'all-times-during-construction, -It-is <br /> - <br /> expected that the Shell Project Manager, Head Operator, HES, and Terminal Mechanic <br /> will be present on the morning of first day for the Pre-Job/Kick Off meeting. Brinderson <br /> shall provide all work forme employees required to conduct the project also be in the Pre- <br /> Job/Kick Off meeting to review training certificates, and job specific certifications. This <br /> will also include the Shell Pre-Job orientation and any safe work practices pertaining to <br /> the work. Emphasis will be placed on communication, Stop Work Authority, observation: <br /> of- changing-work conditions requiring-a-new permit, observance of ambient weather <br /> conditions, what is to be expected of the Contractor, and what is expected of Shell . <br /> Contracxor may be asked to perform weekly safety audit, of all work sites pendingjob <br /> scope. This request will be determined during the Pre-Job meeting. <br /> PROJECT LIFT PLAN , <br /> The_proje.ets. Contact.Water and UCS 6- Removal will both require. the_ use. of a- <br /> reach lift. No slip on attachments will be used per Shell lifting and hoisting procedures. <br /> The crew will review the lifting and hoisting documents before any lifts are done. The <br /> piping sections are no more than 2000 pounds. We will be using an 8 ,000 to 10,000 LB <br /> reach lift to lift them and move them in the laydown area. We will hand transport piping <br /> on to the existing rack in the tank f irm. The three-inch pipe will-be fabricated in 20-foot <br /> sections . The 20-foot sections will allow three people to carry them. Once in the rack the <br /> 3"pipe will be flange connections. The 2" inch will also be installed in 20 ' sections. The <br /> 2" pipe will have welded coupler connections . Brinderson will build fire blanket <br /> protection areas around weld area, obtain hot work permit, gas monitor the area. There <br /> wiP4 bee. qualified operator with a ccrtificationr on the reach lift. There will be a spotter <br /> with'-the-equipmehf atali-time it is operation. The ttype-ofuggingto be used-will- e <br /> Nylon Slings and steel shackles. Each pick is unique. Each pipe spool may require <br /> different rigging techniques, shackles or a nylon sling choke. All rigging will be <br /> inspected before use. All rigging will have a legible weight rating chart. All rigging will <br /> be kept clean and dry. All rigging will be protected from sharp edges or heat. The area <br /> will be observed prior to lift for overhead obstructions.
