SHELL SSTOCKTON TERMINAL <br /> October 24,, 2019 <br /> Site Safety Flan <br /> No person may cross under suspended load ibis includes reach lift mask acid boom. <br /> Equipment will be inspected by qualified operator each day prior to lift. A boom truss <br /> attachment will be used on the reach lift. No slip-on attachment will be used. All load <br /> charts will be-reviewed-prior-to lift: Each-lift will be-on the-daily JHA. Before-each-lift <br /> there will be a job huddle to review JHA and go over each person' s roles and <br /> responsibilities. Before each lift weight will be assessed and load chart will be reviewed. <br /> PROJECT WORD PLAN* <br /> Pothole excavations for theT contactwater project: (Hand digging) will be started after 811 ' <br /> has been called and underground piping identified and marked. We will use soft digging <br /> techniques which include smooth bucket. Hand digging and probing. Excavations will be <br /> shored. UG 6 will be shored with an aluminum trench box . Pump 34 sump will be shored <br /> with aluminum speed shores. Excavations will be confined space entry only. Entry and <br /> exit will be by approved trench ladders . Entrants will wear a harness . Brinderson <br /> employees are not rescue trained, in the event that an entrant needs rescue the hole watch <br /> will ask Shell-Contact to call fire department: The fire departmenttwill-be notified before <br /> entry that confined space work is being performed. No entry will be done into any <br /> excavation over 9 ' 8 " or that has any water accumulation. <br /> Drain Downs for Contact Water Tie Ins will be done using a vac-buck. All LOTO will be <br /> done prior to drain downs. Operations, Crew, and Safety Inspector will review P&ID ' s. <br /> Operations will isolate the system with the assistance from the Brinderson crew. <br /> Operations will -place their locks on the isolation points. Brinderson Foreman will place a <br /> lock on each isolation point. All Brinderson Foreman ' s Keys will be placed in alock box. <br /> Each Brinderson Crew member working on the project will place his or her lock on the <br /> lock box. The .process will be reversed once the work is complete and the Inspector and <br /> Operations has approved the work. <br /> Fire blanket and are shields will be used to protect equipment and people from arc flash. <br /> All hot work will be permitted by an authorized Shell employee. All hot work will be <br /> done with a Brinderson Fire Watch in Safe work areas and in non-safe work areas; <br /> All Shells Life Saving Rules will be followed <br /> Any first aids needs will be reported to operations and Shell Safety Inspector <br /> immediately. Brinderson Foreman will contact Onsite Health and Safety 510-245 -2700. <br /> Onsite will send a technician to site to evaluate and administer first aid. For injuries that <br /> are above first aid level the employee may be transported to the -company doctor or the <br /> local hospital : The type of injury will dictate which establishment-the employee will be <br /> sent to .
