The <br /> following requested information is enclosed: <br /> • Additional application fee of$89.00. <br /> • Completed Masterfile Record Information Form. <br /> • Completed California Licensed Contractor Questionnaire. <br /> A verbal encroachment permit for the off-site monitoring well has been issued by the City of <br /> Tracy. Mark Thrailkill with the City of Tracy can supply encroachment permit details. He <br /> can be contacted at (209) 831-4448. The Site Safety and Health Plan will be forwarded to you <br /> on or before February 3, 1997. <br /> The monitoring well installation is tentatively scheduled for February 4-7, 1997.
