3. Scope of Work <br /> ' 3. SCOPE OF WORK <br /> 1 The following scope of work was completed to accomplish the well destructions: <br /> ' • Obtained the Corporation Grant Deed between G-P and Stewart-Walker Company <br /> (Appendix D) <br /> ' • Obtained Access Agreements between three private residences and G-P to allow access to <br /> their property (Appendix E). The City has maintained a public utility easement corridor <br /> within the residences property <br /> • Obtained an Encroachment Permit for those wells within the City's basin, landscaped and <br /> sidewalk areas, and along the public utility easement corridor (Appendix F) <br /> • Obtained an Access Agreement between G-P and The West Side Irrigation District <br /> (Appendix G) <br /> • Obtained Well & Boring Permit Applications (Appendix H) for well destructions from <br /> San Joaquin County <br /> • Renewed AME's City business license (Appendix I) <br /> • Obtained a fire hydrant permit from the City (Appendix J) <br /> • Marked proposed well destruction locations with white paint for subsequent utility <br /> ' locating <br /> • Contacted Underground Services Alert to obtain notification ticket numbers and request <br /> ' utility locating and marking by private and municipal utilities operators <br /> • Performed a geophysical utility survey of immediate areas within 5 feet diameter of each <br /> ' proposed well destruction to confirm or deny the presence of utilities located and marked <br /> by private and municipal utility operators <br /> ' • Conducted brush and limb removal activities to provide access and work areas for the <br /> drill rigs and crews <br /> ' • Conducted cyclone fencing removal along north boundary of the City's basin and along <br /> The West Side Irrigation District property to gain access to well heads <br /> ' • Notified the RWQCB and San Joaquin County of scheduled activities <br /> • Notified Consolidated Container Company (CCC)personnel of scheduled activities <br /> ' • Received eight 20-yard roll-off bins to containerize the well destruction debris and the <br /> soil and water derived wastes <br /> ' Report on Well Destruction G-P Former Tracy Chemical Packaging Plant <br /> UA2011 Letters and Reports\16014.01\Reports\Well Destruction\Report.doe 75 W.Valpico Road,Tracy,California <br /> ©2011.Georgia-Pacific LLC.All rights reserved. <br /> ' 3 <br /> I <br />
