Van Buren Stockton, California Stockton,California Page 17 X BID REQUIREMENTS This project must be managed by an appropriately qualified California Registered Geologist (RG) with at least 5 years documented continuous experience in UST site investigations. Documentation to this effect, with references, must be provided with your bid. We will require a copy of the 40-hour OSHA approved Hazardous Waste Operation training certificates for all parties on site conducting the investigation as well as copies of the licenses being utilized for this project by the contractor and its subcontractors. All costs for workplans, health and safety plans, permits, outside contractors, sampling, sample analyses, project management, report preparation and all associated costs must be included in your bid. Bids which are judged as incomplete with respect to work effort, costing or technical competency will be considered unresponsive to the bid specifications and disallowed. Bids must be complete as proffered. Should a potential respondent require clarification, questions may be presented to Jeffrey B. Setness at 209/956- 9940. Bids must consist of a technical proposal with justification, detailed cost estimate, contract documentation (including limitations, if any) and must be signed by a California licensed geologist. The successful bidder will be contacted by telephone to arrange final negotiations for contract award. In the event that a contract cannot be negotiated, the next most responsive bidder will be contacted and the process repeated until a contract is awarded or another IFB issued. We reserve the right to reject any and all bids. Field work is anticipated to commence on or about March 15, 1995. The successful bidder will be required upon award to furnish an original Certificate of Insurance naming Hartmann and Setness on a commercial general liability (CGL) and automobile liability coverage in the sum of not less than $1,000,000 per occurrence ($1,000,000 aggregate); and, on a professional liability and pollution liability insurance policy in the sum of not less than $1,000,000 annual aggregate, all of which is on a claims-made basis. The bidder must also show coverage under the worker's compensation laws. Evidence of insurance must be submitted for a bid to be considered responsive. All major subcontractors will be directly contracted to Hartmann and Setness in order to minimize difficulties with reimbursement from the California SB2004 USTCF. All • invoices must be provided with narratives from the consultant in amounts greater than $10,000 so that they can be immediately submitted to the USTCF for reimbursement.
