Van Buren <br /> Stockton, California Stockton,California <br /> Page 17 <br /> X BID REQUIREMENTS <br /> This project must be managed by an appropriately qualified California Registered <br /> Geologist (RG) with at least 5 years documented continuous experience in UST site <br /> investigations. Documentation to this effect, with references, must be provided with your <br /> bid. <br /> We will require a copy of the 40-hour OSHA approved Hazardous Waste Operation <br /> training certificates for all parties on site conducting the investigation as well as copies of <br /> the licenses being utilized for this project by the contractor and its subcontractors. <br /> All costs for workplans, health and safety plans, permits, outside contractors, sampling, <br /> sample analyses, project management, report preparation and all associated costs must be <br /> included in your bid. Bids which are judged as incomplete with respect to work effort, <br /> costing or technical competency will be considered unresponsive to the bid specifications <br /> and disallowed. Bids must be complete as proffered. Should a potential respondent <br /> require clarification, questions may be presented to Jeffrey B. Setness at 209/956- <br /> 9940. <br /> Bids must consist of a technical proposal with justification, detailed cost estimate, <br /> contract documentation (including limitations, if any) and must be signed by a California <br /> licensed geologist. The successful bidder will be contacted by telephone to arrange final <br /> negotiations for contract award. In the event that a contract cannot be negotiated, the next <br /> most responsive bidder will be contacted and the process repeated until a contract is <br /> awarded or another IFB issued. We reserve the right to reject any and all bids. Field <br /> work is anticipated to commence on or about March 15, 1995. <br /> The successful bidder will be required upon award to furnish an original Certificate of <br /> Insurance naming Hartmann and Setness on a commercial general liability (CGL) and <br /> automobile liability coverage in the sum of not less than $1,000,000 per occurrence <br /> ($1,000,000 aggregate); and, on a professional liability and pollution liability insurance <br /> policy in the sum of not less than $1,000,000 annual aggregate, all of which is on a <br /> claims-made basis. The bidder must also show coverage under the worker's <br /> compensation laws. Evidence of insurance must be submitted for a bid to be considered <br /> responsive. <br /> All major subcontractors will be directly contracted to Hartmann and Setness in order to <br /> minimize difficulties with reimbursement from the California SB2004 USTCF. All <br /> • invoices must be provided with narratives from the consultant in amounts greater than <br /> $10,000 so that they can be immediately submitted to the USTCF for reimbursement. <br />
