reports shall contain the information listed in item <br /> II.G.12 above. <br /> 14. Other Information <br /> The owner or operator shall promptly submit all facts or <br /> information which have been omitted or which correct <br /> information in' the permit application or any other report <br /> submitted to the Department. <br /> H. Signatory Requirement <br /> All reports or other information requested by the Department <br /> shall be signed by the owner or operator. For a corporation, <br /> this would be a responsible corporate officer, for a partnership <br /> or sole proprietorship, by a general partner, or the proprietor, <br /> respectively, and for a municipality or other public agency by a <br /> principal executive officer or ranking official. The person <br /> signing the document shall make the following certification: <br /> "I certify under penalty of law that this document and <br /> all attachments were prepared under my direction or <br /> supervision in accordance with a system designed to <br /> assure that qualified personnel properly gather and <br /> evaluate the information submitted. Based on my <br /> inquiry of the person or persons who manage the <br /> system, or those persons directly responsible for <br /> gathering the information, the information submitted <br /> is, to the best of my knowledge and belief, true, <br /> accurate, and complete. I am aware that there are <br /> significant penalties for submitting false <br /> information, including the possibility of fine and <br /> imprisonment for knowing violations." <br /> I. Certification of Construction <br /> The owner or operator may not commence treatment, storage, or <br /> transfer of hazardous wastes at the facility or modified portion <br /> of the facility until: <br /> 1. The owner or operator has submitted to the Department by <br /> certified mail or hand delivery a letter signed by the <br /> owner or operator and an appropriate engineer registered in <br /> California stating that the facility has been constructed <br /> in compliance with the permit; and <br /> 2. The Department has inspected the constructed facility and <br /> finds it is in compliance with the conditions of the <br /> permit; or <br /> 3. The Department has either waived the inspection or has not <br /> within 15 days notified the owner or operator or its intent <br /> to inspect. <br /> -7- <br />
