12. 24-Hour Reporting <br /> The owner or operator shall report to the Department any <br /> noncompliance which may endanger health or the environment. <br /> Any information shall be provided verbally within 24 hours <br /> from the time the owner or operator becomes aware of the <br /> the noncompliance. The following shall be included as <br /> information which must be reported verballyt <br /> within 24 hours <br /> to the Toxic Substances Control Division, Northern <br /> California Section at (916) 739-3145. <br /> concerning release o <br /> f any hazardous waste <br /> a. Information g Y <br /> that may cause an endangerment to public drinking <br /> water supplies. <br /> b. Any information of a release or discharge of hazardous <br /> waste, or of a fire or explosion from the facility, <br /> which could threaten the environment or human health <br /> outside the facility. The description of the <br /> occurrence and its cause shall include: <br /> (1) Name, address, and telephone number of the owner <br /> or operator; <br /> (2) Name, address, and telephone number of facility; <br /> (3) Date, time, and type of incident; <br /> (4) Name and quantity of material(s) involved; <br /> (5) The extent of injuries, if any; <br /> (6) An assessment of actual or potential hazard to <br /> the environment and human health outside the <br /> facility, where this is applicable; and <br /> (7) Estimated quantity and disposition of recovered <br /> material that resulted from the incident. <br /> A written submission shall also be provided within <br /> five days of the time the owner or operator becomes <br /> aware of the circumstances. The written submission <br /> shall contain a description of the noncompliance and <br /> its cause; the periods of noncompliance ( including <br /> exact dates and times) , and if the noncompliance has <br /> not been corrected, the anticipated time it is <br /> expected to continue; and steps taken or planned to <br /> reduce, eliminate, and prevent reoccurrence of the <br /> noncompliance. <br /> 13. Other Noncompliance <br /> The owner or operator shall report all other instances of <br /> noncompliance not otherwise required to be reported at the <br /> time monitoring or other reports are submitted.
