Violation 303, CCR 66265.33. Failed to maintain all communication or alarm systems, spill control, or
<br />decontamination equipment. This is a repeat violation.
<br />Facility did not maintain fire protection equipment (outdated fire extinguishers found), spill control equipment
<br />not replenished, and decontamination equipment not provided as required.
<br />All facility communications or alarm systems, fire protection equipment, spill control equipment, and
<br />decontamination equipment, where required, shall be tested and maintained as necessary to assure its proper
<br />operation in time of emergency.
<br />Provide allproperly maintained required emergency equipment by June 3, 2O16
<br />Violation 304, CCR 66265.35. Failed to maintain adequate aisle space for unobstructed response to
<br />emergencies.
<br />Facility had row upon row of 55 -gallon chemical drums crowded together making it impossible to provide
<br />adequate aisle space to respond to emergencies.
<br />The owner or operator shall maintain aisle space to allow the unobstructed movement of personnel, fire
<br />protection equipment, spill control equipment, and decontamination equipment to any area of facility
<br />operation in an emergency, unless it can be demonstrated to the Department that aisle space is not needed
<br />for any of these purposes.

<br />Violation 305, CCR 66265.17(a . Failed to protect containers from sources of ignition or reaction.
<br />Facility had one 55 -gallon drum labeled PEG011 with a bulging lid indicating some type of reaction was
<br />occurring inside the drum.
<br />(a) The owner or operator shall take precautions to prevent accidental ignition or reaction of ignitable or
<br />reactive waste. This waste shall be separated and protected from sources of ignition or reaction including but
<br />not limited to: Open flames, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static,
<br />electrical, or mechanical), spontaneous ignition (e.g., from heat -producing chemical reactions), and radiant
<br />heat. While ignitable or reactive waste is being handled, the owner or operator shall confine smoking and
<br />open flame to specially designated locations. "No Smoking" signs shall be conspicuously placed wherever
<br />there is a hazard from ignitable or reactive waste.
<br />(b) Where specifically required by other sections of this chapter, the transfer, treatment, storage, or disposal
<br />of ignitable or reactive waste, and the mixture or commingling of incompatible wastes, or incompatible wastes
<br />and materials, shall be conducted so that it does not:
<br />(1) generate extreme heat or pressure, fire or explosion, or violent reaction;
<br />(2) produce uncontrolled toxic mists, fumes, dusts, or gases in sufficient quantities to threaten human health
<br />or the environment;
<br />(3) produce uncontrolled flammable fumes or gases in sufficient quantities to pose a risk of fire or explosions;
<br />(4) damage the structural integrity of the device or facility containing the waste; or
<br />(5) through other like means threaten human health or the environment.
<br />(c) When required to comply with subsection (a) or (b) of this section, the owner or operator shall document
<br />that compliance. This documentation may be based on references to published scientific or engineering
<br />literature, data from trial tests (e.g., bench scale or pilot scale tests), waste analyses (as specified in section
<br />66265.13) or the results of the treatment of similar wastes by similar treatment processes and under similar
<br />operating conditions.
