This version supersedes the previous <br />Checklist Only provided on 611812014. <br />(Violation continues from Page 4) <br />Observed at the tab in Building A: <br />• Multiple containers on two lab benches, shelves, sink, and on the floor of the lab. Some of the <br />containers were marked as sulfuric acid, T.E.A. (triethanolamine), glycol, ether, detergent, <br />corrosives, coloring agent, sealer, adhesive, solvent, etc., while others have no labels, some of <br />which generator states he "has no idea" (Lab Photo # 1,2,3,4,5,6,7,8,9,10,12,13). <br />• Solid substance on benches and on the floor (Lab Photo #2,7,8). <br />• Spillage on shelves (Lab Photo #11, 12.) <br />Observed outdoor: <br />o Spillage between Building A and B and inside the berms at Building B (Outdoor Photo # 1,2,3,4) <br />from All Purpose Cleaner, red Degreaser, and various mix tanks within the berm area (Outdoor <br />Photo # 5,6,7), and also within and outside of berm of south tank cluster (Outdoor Photo # 1). <br />o Solid waste in drum and bucket (Outdoor Photo # 9,10 <br />o Corrosive solid on piping of tank cluster (Outdoor Photo 11, 12) and within berm area of corrosive <br />tank (Outdoor Photo 13) <br />o Unknown substance in buckets, bottles, and drums at the tank cluster area (Outdoor Photo #14, <br />15, 16, 17, 18), and multiple bottles on palettes (Outdoor Photo #19, 20,21) which were <br />previously observed during the August 2013 inspection but had yet to return to compliance (see <br />Violation #108 of the routine hazardous waste inspection report dated August 30, 2013 and <br />associated photos). <br />o Spillage on shelves (Lab Photo #11, 12.) <br />Immediately make a hazardous waste determination of the above wastes and manage each accordingly, <br />he waste as hazardous waste in accordance to California Code of Regulations (CCR) Title <br />or dispose of the <br />22. If you determined the waste to be a non -hazardous, please provide to EHD the rationale and <br />evidence (i.e. third party laboratory testing results) supporting such a determination. Submit a statement <br />and supporting documentation explaining how this waste was managed by 9/8/2014. <br />116. Failed to provide annual review of initial training for personnel the last three years, nor has <br />generator provided any documentation to meet return to compliance for the same violation cited in the <br />routine hazardous waste inspection report dated August 30, 2013.
