This version supersedes the previous <br />Checklist Only provided on 611812014. <br />703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br />least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br />emergency coordinator and post the required information by a phone. A form has been provided to you <br />during this inspection that can be used for this purpose. Submit proof of correction to the EHD by <br />9/8/2014. <br />804, 806, 808, 809. Of the violations cited in the routine hazardous waste inspection report dated <br />August 30, 2013, pertaining to the one Universal Waste fluorescent lamp and electronic waste observed <br />at the repair room, and electroniG wastes not stored in closed containers nor labeled at the product <br />storage room at northwest of the stairwell, generator has not returned to compliance. Universal Wastes, <br />including aerosol cans, lamps, alkaline batteries, mercury containing switches, CRT, electronic devices, <br />etc. shall be stored in a. closed container that prevents breakage and release to the environment, and <br />the waste containers shall be marked to identify the type of Universal Waste and demonstrated the initial <br />date of accumulation. Universal waste shall be managed within one year of storage, and a copy of the <br />disposal receipt for the waste be retained on site for at least 3 years. Immediately dispose of the above <br />Universal Waste lamps and electronic waste in accordance to Title 23, and submit a copy of the disposal <br />receipt to EHD by 9/8/2014. <br />812. Disposal records of universal wastes were not kept on site. Facility shall retain a copy of each <br />disposal record of universal waste including but not limited to lamps, alkaline batteries, mercury <br />containing switches, CRT, and electronic devices, for at least 3 years available for inspection. <br />Immediately locate and submit a copy of all Universal Waste disposal records. for the last three years to <br />EHD by 9/8/2014, and begin maintaining all such records on site in the future per this section. <br />999. HSC 25404 (e)(4). Electronic submittal requirement on the California Environmental Reporting <br />System: (CERS) has not been met. Facility shall submit electronic information for hazardous materials <br />management system, including hazardous waste, on the CERS website, which can be found on http:// <br />cers.calepa.ca.gov/.
