(c) Facility personnel shall take part in an annual review of the initial training required in subsection (a) of this <br /> section. <br /> A checklist outlining the requirements for Personnel Training Requirements for Hazardous Waste Generators <br /> was provided to the owner by email on October 30, 2015. <br /> Train all employees who handle hazardous waste in hazardous waste management and provide <br /> updated employee training to this office by June 3, 2016. <br /> Violation 111, CCR 66265.16(d). Failed to maintain complete personnel training records on hazardous <br /> waste management and emergency response procedures. <br /> Complete training records were not available for all hazardous waste employees. <br /> (d) The owner or operator shall maintain the following documents and records at the facility: <br /> (1) the job title for each position at the facility related to hazardous waste management, and the name of the <br /> employee filling each job; <br /> (2) a written job description for each position listed under subsection (d)(1) of this section. This description <br /> may be consistent in its degree of specificity with descriptions for other similar positions in the same company <br /> location or bargaining unit, but shall include the requisite skill, education, or other qualifications, and duties of <br /> facility personnel assigned to each position; <br /> (3) a written description of the type and amount of both introductory and continuing training that will be <br /> given to each person filling a position listed under subsection (d)(1) of this section; <br /> (4) records that document that the training or job experience required under subsections (a), (b), and (c) of <br /> this section has been given to, and completed by, facility personnel. <br /> Train all employees who handle hazardous waste in hazardous waste management and provide <br /> updated employee training to this office by June 3, 2016. <br /> Violation 112, CCR 66262.20. Failed to prepare manifests for transportation of hazardous waste. <br /> Facility owner stated hazardous waste was transported from his facility at 2000 Sanguinetti to this location by <br /> his employees. <br /> A generator who transports, or offers for transport a hazardous waste for off-site transfer, treatment, <br /> storage, or disposal, shall prepare a Uniform Hazardous Waste Manifest (OMB Control number 2050-0039) on <br /> EPA Form 8700-22, and, if necessary, a Continuation Sheet on EPA Form 8700- 22A, according to the <br /> instructions included in the Appendix to chapter 12 of this division before the waste is transported off-site. <br /> Immediately,prepare a hazardous waste manifest for all hazardous waste. <br /> Violation 117, CCR 66268.7(x). Failed to make determination for land disposal. <br /> Facility had hazardous waste identified as RCRA waste D001 mineral spirits transported on August 6, 2014 <br /> manifest #01127977133K with no Land Disposal Restriction (LDR) document available for review. <br /> Requirements for generators: <br /> (1) A generator of hazardous waste shall determine if the waste has to be treated before it can be land <br /> disposed. This is done by determining if the hazardous waste meets the treatment standards in article 4 or <br /> article 11 of this chapter. This determination can be made concurrently with the hazardous waste <br /> determination required in section 66262.11 of this division, in either of two ways: testing the waste or using <br /> knowledge of the waste. If the generator tests the waste, testing would normally determine the total <br /> concentration of hazardous constituents, or the concentration of hazardous constituents in an extract of the <br />
