An emergency coordinator was not listed in the contingency plan. The <br /> contingency plan must include: <br /> 1. Description of actions facility personnel will take in response to fires,explosions, or any sudden or <br /> non-sudden release of hazardous waste to air, soil or surface water at the facility; <br /> 2. Description of arrangements made with local police departments, fire departments, hospitals,contractors and <br /> State and local emergency response teams to coordinate emergency services,- <br /> 3. List of names, addresses, and phone numbers(office/home/cell)of all persons qualified to act as emergency <br /> coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and <br /> all others in order in which they will assume responsibilities; <br /> 4. List all emergency equipment,which includes location, physical description, and brief outline of its capabilities-, <br /> 5. Evacuation routes for facility personnel where there is a possibility of evacuation; <br /> 6. Current telephone number for State Office of Emergency Services. <br /> If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency <br /> plan,the owner or operator need only amend that plan to incorporate hazardous waste management provisions that <br /> are sufficient to comply with the requirements of this chapter. Immediately prepare or amend an existing plan to <br /> fulfill the requirements of this chapter. Submit a copy of the contingency plan to the EHD. <br /> This is a repeat violation, Class 11. <br /> 114 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Manifest number 005388283JJK from August 24,2016 was found without a signed off copy from the destination <br /> facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site <br /> for three years and have them readily available for review. If the generator did not receive a copy of the manifest <br /> with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted <br /> within 60 days of the date the waste was accepted by the initial transporter,the generator shall submit a legible copy <br /> of the missing manifest,with some indication that the generator has not received confirmation of delivery. This <br /> information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O.
