Stockton, CA 95210 <br />FILE COPY <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Margaret Lagorio, REHS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />San Joaquin County Environmental Health Department (EHD) performed a routine hazardous waste inspection <br />March 19, 2008 at the above referenced site. <br />An inspection report was issued identifying information to be submitted to bring this site into compliance. This <br />information was required to be submitted by April 19, 2008. A response was submitted by Pep Boys to the EHD on <br />April 29, 2008. The following information has not been received or is incomplete resulting in a non-compliant status <br />for this facility. <br />#46: Hazardous Waste Contingency Plan. The contingency plan submitted to the EHD does not include an <br />evacuation map, list of emergency equipment or list of local authority arrangements. A "Hazardous Waste <br />Contingency Plan" form is included with this letter. <br />#64: Hazardous Waste Tank Assessment. The EHD has not received the tank assessment document. The EHD is not <br />granting exemptions from this requirement. Guidance documents are included with this letter. <br />#69: Hazardous Waste Tank daily inspection records. The inspection records submitted to the EHD are not <br />complete. See Title 22 California Code of Regulations section 66265.195 for the inspection requirements. A copy of <br />this section is included with this letter. <br />Return to Compliance Certification: Send this document to the EHD when all violations are resolved. The form is <br />included with this letter. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($115).
