Records available do not indicate hazardous waste training has been conducted for every <br /> employee in the last year. According to the training records on site, the shop mechanics were last <br /> trained on June 29, 2012. The hazardous waste training program shall be completed by each employee <br /> within six months of hire, prior to unsupervised work, and shall be reviewed at least annually. <br /> Immediately provide current training as necessary for each employee and ensure that employees <br /> receive this training annually. Submit a copy of the updated training records to the EHD by December <br /> 18, 2013. <br /> 117. The training records on site were incomplete. The training records for clerical and management <br /> only include a record that each employee was trained. They do not include a description of the training <br /> to be given to each employee, job titles, or written job descriptions. Facility personnel shall complete a <br /> training program that teaches them to perform their duties in a way that ensures the facility's compliance <br /> with Title 22 regulations. This program shall be directed by a person trained in hazardous waste <br /> management procedures. The training program shall be completed by each employee within six months <br /> of hire, prior to unsupervised work, and shall be reviewed annually. Training records shall be maintained <br /> on site until closure of the facility and shall include: <br /> - name &job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately update the training records to contain all of the required information and ensure that all <br /> employees complete this training within six months of hire and at least annually thereafter. Submit a <br /> copy of the updated training program and records to the EHD by December 18, 2013. <br /> REPEAT VIOLATION <br /> 211. According to Mr. Garth Williams, aerosol cans, including spray paint cans, are crushed in the oil <br /> filter crusher and all liquids from the cans are placed in with the used oil. No person who generates, <br /> stores, or transfers used oil shall intentionally contaminate used oil with other hazardous waste. <br /> Immediately stop disposal of aerosol can wastes with the used oil. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: ` .
