t <br /> (4) Sample or monitor, at reasonable times, for the purposes <br /> of assuring permit compliance or, as otherwise authorized <br /> by law, any substances or parameters at any location. <br /> (h) Monitoring and Records <br /> (1) Samples taken for the purpose of monitoring shall be <br /> representative of the monitored activity. Laboratory <br /> and sampling methods must be those specified in the <br /> approved operation plan. <br /> (2) The owner or operator shall retain records of all moni- <br /> toring information, copies of all reports and records <br /> required by this permit, and records of all data used to <br /> complete the application for this permit for a period of <br /> at least three years from the date of the sample, measure- <br /> ment, report, or record. These periods may be extended <br /> by request of the Department at any time and are auto- <br /> matically extended during the course of any unresolved <br /> enforcement action regarding this facility. <br /> (3) Records of monitoring information shall include: <br /> (i) The date, exact place, and time of sampling or <br /> measurements; <br /> (ii) The individual(s) who performed the sampling or <br /> measurements; <br /> (iii) The date(s) analyses were performed; <br /> (iv) The individual(s) who performed the analyses; <br /> (v) The analytical techniques or methods used; and <br /> (vi) The results of such analyses. <br /> (i) Planned Changes <br /> The owner or operator shall receive Department approval prior <br /> to any planned physical alterations or additions to the <br /> permitted facility. <br /> (j) Anticipated Noncompliance <br /> The owner or operator shall give advance notice to the Depart- <br /> ment of any planned changes in the permitted facility or <br /> activity which may result in noncompliance with permit <br /> requirements. <br /> -5- <br />
