(k) 24-Hour Reporting The owner or operator shall report to the Department any noncompliance which may endanger health or the environment. Any information shall be provided within 24 hours from the time the owner or operator becomes aware of the noncompliance. The following shall be included as information which must be reported within 24 hours: (1) Information concerning release of any hazardous waste that may cause an endangerment to public drinking water supplies. (2) Any information of a release or discharge of hazardous waste, or of a fire or explosion from the facility, which could threaten the environment or human health outside the facility. The description of the occurrence and its cause shall include: (i) Name, address, and telephone number of the owner or operator; (ii) Name, address, and telephone number of facility; (iii) Date, time, and type of incident; (iv) Name and quantity of material(s) involved; (v) The extent of injuries, if any; (vi) An assessment of actual or potential hazard to the environment and human health outside the facility, where this is applicable; and (vii) Estimated quantity and disposition of recovered material that resulted from the incident. A written submission shall also be provided within 15 days of the time the owner or operator becomes aware of the circum- stances. The written submission shall contain a description of the noncompliance and its cause; the periods of noncompli- ance (including exact dates and times) , and if the noncompli- ance has not been corrected, the anticipated time it is expected to continue; steps taken to mitigate the noncompli- ance; and steps taken or planned to reduce, eliminate, and prevent reoccurrence of the noncompliance.
