1/29/04 Filed loose sanitary sewer documents. <br /> (hand written results from DPI) JJ <br /> 10/10/03 Filed inspection from the City of Stockton. (Fire Marshall) JJ <br /> Received a call back from Doug Baker I asked him to fax me back <br /> sampling results for the last 3 months of the water sampling being <br /> taken @ the sanitary sewer(3 months x 2samples per month) also <br /> informed him that he ne - ' '' nr fax n rnn u of <br /> . JJ <br /> 8/0503 Placed a call to DPI and left a message with the secretary to have <br /> DougBaker to call me back JJ <br /> 7/31/03 Placed a call to DPI and asked if DPI had submitted data to DTSC <br /> about cyanide rinse water. Doug Baker of DPI stated that he had a <br /> test company come out and that they where waiting for the test <br /> results. I <br /> Doug Baker said that <br /> he was in the process of hiring o do this. JJ <br /> 7/31/03 Placed a call tp Asha Arora DTSC Cyanide order contact she told <br /> p o $ e t at she had received an application but that she was waiting on i (AA v�q <br /> 0 test results of the cyanide wash water before tx. She also told me 5�(r� <br /> that the inspector for the site was going to be Robert Brushia (916) ye4. <br /> 255-3781. <br /> I then called him and left him a message asking him if I could come JJ <br /> along with his inspection. <br /> /31/03 Filed Cert of Return to Compliance JJ <br /> 7/31 Filed DTSC Report and Sample results. All 3 samples were hot <br /> see Report. JJ <br /> 7/31/03 Placed a call to Asha Aurora of DTSC to check if DPI had applied <br /> for a cyanide order. She said that they have. JJ <br /> 6/16/03 Sent fax to Doug Baker(948-2503) contained cyanide order <br /> documentation also asked for Phase I Fnviramn ntal Assessment JJ <br /> documentation. <br /> 6/16/03 Received call from Doug Baker. He had questions about how to <br /> comply with inspection issues. I informed him that there appeared <br /> to be no record of a Phase I Environmental Assessment, or <br /> documentation supporting the authorization for the treatment of <br /> cyanide. I explained that the treatment of cyanide required an order <br /> from DTSC. I recommended that he assess the situation and deal <br /> with it appropriately. JJ <br /> 6/11/03 Faxed a copy of the notification form from 1995. I was unable to <br /> find and fax any type of tiered permitting authorization <br /> documentation. I have requested that Violetta Mislang mail proper <br /> documentation to DPI so they can get the ball rolling on a Phase I <br /> Environmental Assessment(PEA).
