JJ <br /> 5/20/03 Today I went to Delta Plating and produced both copies of <br /> Hazardous waste and Tiered Permitting reports for Doug Baker. I <br /> discussed the findings with him and discussed the photos one by one <br /> to enshure he understood the problems in them. I also discussed the <br /> time limitations and expectations and expectations for compliance <br /> of specific issues. At the end of the inspection review Doug was <br /> encouraged to ask any questions. Another card was left on site. JJ <br /> 5/15/04 While discussing the site with Doug Wilson- Supervising REHS it <br /> was brought to my attention that an OSHA Referal was in order for <br /> the poor housekeeping,possible cyanide gas issues, and possible <br /> confined space issues. The referal was made today JJ <br /> 5/13/03 Today Leo Valdez and I went out to the site to conduct a paper <br /> inspection. We found many of the documents we were looking for, <br /> but were unable to find a(1)PEA(2) financial insurance data(3) a <br /> closure plan for the unit, (4) DTSC form 1772, and the State <br /> approval letter from a certified engineer. We asked Doug Baker to <br /> look these items up and fax them to (209) 468-3433. At this time I <br /> left matted copies of photographs along with commentary with <br /> Doug Baker. JJ <br /> 5/06/03 Today I conducted part of the inspection(Tiered Permitting) at the <br /> site. The inspection was conducted by Kasey Foley- Supervising <br /> REHS, Leo Valdez Hazardous Substance Scientist DTSC. Mike <br /> Tucker took us through the facility after placing a call to his <br /> supervisor Doug Baker. Doug Baker granted approval for the <br /> Inspection. At the time of the inspection we were unable to look <br /> through the documentation because the person in charge of filing <br /> these document was at home recovering from surgery. We were <br /> told that if we came back next week Doug Baker would be able to <br /> provide the needed documents. We toured the facility taking <br /> pictures. Samples were also taken from the buffer wheel floor <br /> sweep,polishing wheel floor sweep and metal salt from the large <br /> N� hazardous waste storage tank. Duplicate samples were left on site <br /> ` qtr and a chain of custody was given to th dra" along with <br /> r� r�o <br /> the les. Mr. Baker did not have a copy of the <br /> 8-27-99inspection prior to the 11-17-00 inspection. <br /> 11/21/00 Added CA unit to site program list and mailed PBR compliance <br /> information for Closure/Financial Responsibility.
