description for each position related to hazardous waste <br /> management. <br /> Violation #9: <br /> Within 10 days of receipt of this report, personnel <br /> training records shall be updated with a written <br /> description of the type and amount of both introductory <br /> and continuing training that is given to each person <br /> filling a position related to hazardous waste management. <br /> Violation #10: <br /> Effective immediately, the facility shall be maintained <br /> and operated to minimize the possibility of a fire, <br /> explosion, or any unplanned sudden or non-sudden release <br /> of hazardous waste or hazardous waste constituents to the <br /> air, soil, or surface water which could threaten human <br /> health or the environment. <br /> Violation #11: <br /> Within 10 days of receipt of this report, the internal <br /> alarm shall be available for use whenever hazardous waste <br /> is being poured, mixed, spread, or otherwise handled. <br /> Violation #12 : <br /> Within 10 days of receipt of this report, emergency spill <br /> control equipment and decontamination equipment shall be <br /> available in the hazardous waste treatment and storage <br /> area. <br /> Violation #13 and #14: <br /> Effective immediately, all containers of hazardous waste <br /> shall be closed except when adding or emptying waste. <br /> When all violations have been corrected, please send <br /> written certification to this office at the letterhead <br /> address. This certification shall contain the sample <br /> certification statement indicated on Attachment 1. <br /> PHS-EHD may conduct a reinspection of your property to verify <br /> compliance. The issuance of the Report of Violation(s) and <br /> Schedule of Correction does not preclude PHS-EHD from taking <br /> any administrative, civil or criminal action as a result of <br /> the violations noted herein. <br /> OBSERVATIONS MADE DURING MAY 19 , 1995 INSPECTION <br /> 1. Records will be inspected on May 26, 1995. <br /> 2. Clean up of polishing, burnishing dusty waste is not being <br /> done. DPI has plastic drums with screens sitting on top. <br /> However, there is just a residue of dust inside drum. <br />
