DISCUSSION WITH MANAGEMENT: <br /> After discussing violations with Mr. Baker, Ms. Violett <br /> and I departed. <br /> ADDENDUM <br /> On October 25, 1992, I inspected paperwork with Mr. Baker. <br /> DPI generated 28.0 tons of hazardous waste in 1991. DPI <br /> has kept a timekeeping log of inspections performed in the <br /> hazardous waste area and inspections of the equipment. <br /> DPI received training by Nationwide Insurance on July 12, <br /> 1991 according to Mr. Baker. There was no documentation <br /> of this training. Mr. Baker admitted employee training <br /> was not documented in employee records. <br /> Finally, Mr. Baker took me on a short inspection where I <br /> reviewed the safety gear box. The gear box was dusty, <br /> equipment inadequate, there was no alarm in the plating <br /> room. Mr. Baker stated he could go to a 5 yard bulk <br /> transport bin for storage of the filter cake.
