REMINDER: Ask StrongPak or PSC for an explanation if you do not <br />understand anything completed on the manifest. <br />You must keep a copy of the manifest at the store in the EH&S Program <br />Binder. This manifest copy should be completed through Item 17. The <br />StrongPak technician will also leave you with a completed LDR. <br />Once the waste arrives at its final destination for proper disposal or <br />recycling, the TSDF will sign Item 20 and return the final copy of the manifest <br />to the store. It typically takes about 30 days to receive the final manifest in <br />the mail. Your final manifest must be signed on lines 15, 17, and 20 to be <br />complete. <br />If the store does not receive a signed copy of the manifest in the mail within <br />35 days of shipment, you must call StrongPak at 1-770-409-1500 or PSC at 1- <br />866-617-1766. <br />All copies of the manifest must be kept in your store EH&S Program Binder <br />for at least 3 years. If your State requires a manifest copy be submitted to <br />the state environmental agency, StrongPak or PSC will submit the required <br />documentation on CVS/pharmacy's behalf. <br />Emergency CVS Safety Hotline: <br />Information <br />CVS/pharmacy utilizes 3E Company as the primary contact for emergency <br />response, as is listed on the poster. The phone number provided on the <br />Emergency Information poster is the 3E Emergency Response line at 1-800- <br />451-8346, which is staffed by 3E Company 24 hours a day, 7 days a week, <br />365 days a year. <br />Additionally, CVS/pharmacy has a CVS Safety Hotline that is also manned by <br />3E Company and can be reached at 1-888-464-2960.
