Upon initial contact, the colleague will be asked if he or she has a <br />medical emergency or chemical spill. It is important for colleagues to <br />identify if either of these situations is applicable. If neither is <br />applicable, the caller will be asked for the following information to <br />complete the MSDS request process: <br />• Callers facility <br />• Name <br />• Phone number <br />• Fax number <br />• Chemical/Product full label name and any applicable part <br />numbers <br />• Manufacturer <br />3E Company will deliver the requested document to the fax number <br />specified and provide the caller with his or her request number. This <br />request number can be utilized for any follow-up activities related to <br />this particular request. <br />Poison and Exposure Control <br />When a chemical exposure has taken place, the CVS/pharmacy <br />colleague should contact the CVS Safety Hotline for first aid advice. The <br />colleague must advise that he or she has a medical situation, if <br />applicable. If poison control is requested the CVS/pharmacy colleague <br />will be transferred to a poison control specialist for first aid advice. <br />Note: HIPAA protected information will remain confidential. <br />Spill Response <br />3E Company provides 24 hour emergency response cleanup support in <br />the event of a large spill. CVS/pharmacy colleagues should be prepared <br />to provide the following information when contacting 3E Company <br />through the CVS Safety Hotline for spill response assistance: <br />• Caller's facility <br />• Name <br />• Phone number <br />• Injuries or exposures as a result of the release <br />• Chemical/Product full label name and any applicable part <br />numbers/hazard warnings <br />• Manufacturer <br />• Quantity of release <br />• Details of the incident <br />© CVS/pharmacy 16 <br />CONFIDENTIAL BUSINESS INFORMATION <br />CVS000010357 <br />
