Documentation <br />Hazardous When StrongPak picks up any waste at <br />Waste the store, they are required to leave <br />Manifests paper work with the Store Manager. <br />and LDRs They will leave a "Hazardous Waste <br />Manifest" and "Land Disposal <br />Restriction" (LDR) form after each visit <br />to the store. The Manager -on -Duty will <br />be required to sign the manifest and <br />LDR and has received additional <br />training in order to do this. Refer to the <br />EH&S Program Binder for additional <br />details on manifests. <br />Another copy of the manifest should come to the store by mail. This copy <br />should have 3 signatures on it (lines 15, 17, and 20). If the store does not <br />receive a signed copy of the manifest in the mail within 35 days of pick up, the <br />Store Manager must promptly call StrongPak at 770-409-1500. <br />All copies of the manifests and any LDRs must be kept in the EH&S <br />Program Binder for at least three years. <br />Emergency The Pharmacy must have an <br />Information Emergency Information Poster by an <br />Poster out -dialing telephone to allow for <br />easy access in an emergency. <br />Blank posters are provided in the <br />EH&S Program Binder. See your <br />Store Manager for assistance on <br />obtaining a poster and completing it. <br />Handling If you are approached by an Inspector from an Environmental Regulatory <br />Regulatory Agency, you MUST immediately inform the Manager -on -Duty, as he or she <br />Visits has received additional training on how to properly handle a regulatory visit. <br />You MUST NOT answer any questions about this program prior to informing <br />the Manager -on -Duty. <br />© CVS/pharmacy <br />7 <br />11 <br />CONFIDENTIAL BUSINESS INFORMATION <br />CVS000010410 <br />
