(209) 468-3433 Web: www.sjgov.ora/ehd/unitiii.html <br />CONTINUATION FORM <br />Page: 3 of 5 <br />OFFICIAL INSPECTION REPORT <br />Date: 06/30/11 <br />Facility Address: 1885 W. 11th ST Tracy CA <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />Class II Violations: <br />Violation #7 -Failed to determine if a waste is a hazardous waste 66262.11 California Code of Regulations <br />(CCR). <br />At time of inspection, damaged and leaking containers, customer returns, expired products, etc., which may be a hazardous <br />waste, are being sent to the Distribution Center (DC) in Patterson without a hazardous waste determination made. <br />Immediately make a hazardous waste determination before shipping to the DC pursuant to Title 22 hazardous <br />waste regulations and handle accordingly or handle as hazardous waste. Within 30 days provide written <br />documentation describing how waste is being handled. <br />Violation #20 -Failed to properly label containers of hazardous waste 66262.34 (CCR). <br />Violations found in the following areas: <br />Hazardous Waste Storage Area: <br />-Clear plastic "Ox" Tote <br />Missing the start accumulation date <br />-Clear plastic "C -B" Tote (Photo) <br />Missing start accumulation date <br />All containers with hazardous waste must have a label with the following information: <br />The words "Hazardous Waste" <br />Name and address of the facility <br />Composition (contents) <br />Accumulation start date <br />Physical state <br />Hazardous property <br />Immediately properly label all containers storing hazardous waste. <br />Note: All Storage bags had the words "Potentially Hazardous Waste" on them and all Hazardous waste labels <br />had the words "Pending further evaluation" on them. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Re y/ea/d/'`B/fyy:•/�.. -' <br />l6�lj I <br />Title: <br />I <br />EHD 23-02-003 Rev 0 /11 CONTINUATION FORM <br />L____1 <br />
