(209) 468-3433 Web: www.sipov.org/ehd/unitiii.html <br />CONTINUATION FORM <br />Page: 4 of 5 <br />OFFICIAL INSPECTION REPORT <br />Date: 06/30/11 <br />Facility Address: 1885 W. 11 th ST Tracy CA <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />Violation#30-Empty container larger than 5 gallons 66261.7(f). <br />At time of inspection empty 55 gallon drum that once held photo hazardous waste was seen. Empty <br />containers larger than 5 gallons that once held a hazardous material or waste must be marked with the word <br />"empty" and that date it was emptied. The empty container must be managed within 1 year. <br />Immediately manage this containers and begin this practice for future storage of empty containers. <br />50. Stored Hazardous Waste on site greater than 180 days 66262.34(d) CCR. <br />At time of inspection the following containers did not have a start accumulation date. <br />-Clear plastic "Ox" Tote <br />Missing the start accumulation date <br />-Clear plastic "C -B" Tote (Photo) <br />Missing start accumulation date <br />Immediately have these wastes removed by a registered hazardous waste hauler, under manifest, and submit <br />a copy to this office. <br />52. Failed to train all employees on waste handling and emergency procedures per 40 CFR Section 262.34(d) <br />(5)(iii): <br />It was not demonstrated employees are familiar with proper waste handling and emergency procedures, relevant to their <br />responsibilities during normal facility operations and emergencies. <br />Note: The product examples listed on the "Potentially hazardous material management chart" are not clear, <br />nor does it capture all items that are a hazardous waste in a retail store. Thus, leaving the determination of <br />which container the hazardous waste should be placed in, to the employee, which could led to compatibility <br />issues. During the inspection "Wisk" laundry detergent was found in the oxidizer container and the C -B <br />container. <br />Also noted during the inspection was the practice of placing several hazardous waste containers of different <br />waste types in a single bag, and then placing in the hazardous waste container. <br />In the photo area there were 2 hazardous wastes containers, a "ST" container and a "C -B" container. The <br />waste streams in these containers were a mixture of the same wastes which had not been segregated <br />according to the photo hazardous waste chart. <br />Within 30 days, submit copies of hazardous waste training records for employees at this location. Include <br />topics covered during this training. Immediately implement a training procedure for all employees who <br />handle or respond to hazardous waste incidents and submit a copy of the procedure to this office <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Re ived Ely; <br />�Zj <br />Title: <br />T <br />EHD 23-02-003 �.,,,07,/0/11 CONTINUATION FORM <br />
