• Poison Control Center <br />The name, telephone number and location of police, fire and other agencies, as listed <br />above, are noted below. If emergency personnel are called to the site, efforts should be made to <br />accommodate their operations in the support zone. In lieu of this possibility, response personnel <br />may have to enter the exclusion zone. The Site Safety and Health Officer and the Project <br />Manager shall supervise entrance into the exclusion zone so as to prevent injury or detrimental <br />exposure to these personnel. <br />Geofon will have mobile phones onsite for use during normal operations and emergency <br />situations. <br />A first-aid kit will be located in the support zone. The Site Safety and Health Officer will <br />perform minor first aid. All first aid, whether it is major or minor, will be administered in a <br />manner that minimizes exposure to blood and/or body fluids. All first-aid providers will utilize <br />universal precautions when treating injuries. Universal precautions is an approach to infection <br />control that considers all blood and certain body fluids to be treated as if known to be infectious <br />for HIV, HBV, and/or other blood-borne pathogens. Where exposure to blood and/or body <br />fluids occurs, the requirements of CCR, Title 8, Section 5193, "Blood-borne Pathogens", will be <br />followed. If an injured individual requires further attention, the individual will immediately be <br />transported to the nearest hospital. A map illustrating the route to the nearest hospital is included <br />as Attachment 1 and will be maintained onsite. <br />® If possible, the individual will be decontaminated prior to transport to the facility. If <br />the injury is serious, decontamination is of secondary importance. All accidents, <br />without regard to their severity, shall be reported in writing to the Site Safety and <br />Health Officer, within 24 hours. Within two working days of any reportable accident, <br />Geofon shall complete and submit to the COR an accident report on ENG Form 3394 <br />in accordance with AR 385-40 and USACE Supplements to that regulation. <br />21 <br />
