CONTRACTORS QUALITY ATROL REPORT (QCR) SUBMIT DAILY TO TH EPRESENTATIVE <br />DAILY LOG OF CONSTRUCTION - MILITARY l REPORT NUMBER <br />404 PAGE 1 <br />PROJECT DATE <br />TO -0007 Remove 2 UST's. Install 1 AST 11 Jun 2001 <br />DDJC-Sharpe Site. CA CONTRACT NUMBER <br />DACW05-00-D-0008 <br />CONTRACTOR (Or hired labor) 1. WEATHER <br />Geofon Inc_ <br />No Interruption. No Precipitation <br />SUPERVISOR Temperature Min 68F Max 85F <br />2. QC NARRATIVE(S) <br />Subject: General Comments: <br />Geo -fon crew arrived on site at 0600_ We began washing out the UST at 0630. The <br />Evergreen Env_ vacuum truck arrived at 0730. A total of 75 gallons ofrinsate <br />and sludge was vacuumed. Fred Martin and Don Valentine arrived on site at <br />0800_ I reviewed all procedures with them. The dry ice arrived at approx <br />0800. The dry ice was placed into the USTs at approximately 0830_ LEL and 02 <br />readings were taken at 0945 hrs. LEL was O and 02 was 12 on both of the USTs_ <br />Another reading was taken at 1000. LEL was 0 and 02 was 3%_ The manifest was <br />signed by Jeffery Grady (DDJC Env. Office) at approx 1000 hrs_ ECI trucks were <br />on site at 0800 hrs_ I called the crane company (Thompson & Rich Crane <br />Services) at 0900 hrs. The 70 ton crane that was ordered broke down. <br />Arrangements were made to get a 200 ton crane to the site. I met with the <br />county representative (Alison Youngblood) at 0900 hrs_ II told Alison of the <br />crane situation_ I received a phone call from TRC informing me that a crane <br />would arrive at approximately 1000 hrs_ The crane arrived at 0930. Good <br />timing. The crane set itself up for a self check_ An inspection of the crane <br />was made at this time by Fred Martin and Robert Castaneda. All checks were <br />good_ The crane set up on the Northwest end of the UST. The lowbeds were to <br />backup under the UST once given the ok. Other representatives on site included <br />Maurice Benson, Dan Beever and Jeffery Grady from the base. Don Valentine and <br />Fred Martin from the COR. Allison Youngblood from the County. Tasawwar Khan, <br />Robert Castaneda. Jose Jimenez. Mark Swindle. Tariq Sharkoo, and Kuleib <br />Al-Shurei from Geofon Inc_ and Sree Akkenbally from Alterra Consultants to do <br />the sampling_ The 'first UST was lifted at approximately 1007 hrs_ The UST was <br />intact and free from any damage_ The UST was manufatured in the Stockton <br />Manufacturing Company_ Both USTs were double walled steel tanks_ The pea <br />gravel was removed from the bottom and then loaded onto the lowbed. The second <br />UST was loaded at approximately 1040_ Condition was good. no noticable damage_ <br />Good job. The crane truck left the site at approximately 1100 hrs. The <br />manifest for the USTs were signed by Jeff Grady_ The USTs left the site at <br />approximately 1130 hrs. The tank and county referance numbers were painted on <br />the ends of the USTs prior to demobilizing. Sampling began at approx_ 1120 <br />hrs_ Two samples were taken from underneath the pipeline locations_ There <br />were 6 samples taken from the stockpile. These samples were taken under the <br />direction of the County representative. We were not able to get the necessary <br />core samples because of the unstable walls. An attempt was made to get to the <br />west perimeter part of the concrete pad but because of unstable soils we were <br />forced to .quit. A water main was the main concern on the west perimeter of the <br />excavation_ There were obstacles at all four sides of the excavation. After <br />intensive discussion it was decided that we would need to backfill the <br />excavation and request a modification for a boring machine to complete the <br />sampling. All parties were in agreement. Because of the instability of the <br />soil backfilling of the excavation would take place immediately_ I have enough <br />faith that the excavated material results will come back clean. We will <br />therefore use this material for backfill_ We will then place enough base <br />material to remove the hazards from this excavation. The samples were taken by <br />Alterra Consulting to the Fed ex in Manteca at approx 1500 hrs. All samples <br />were immediately placed in an iced container. The county and COR <br />
