WEATHER Geofon Inc. No Interruption, No Precipitation SUPERVISOR Temperature Min SOF Max 73F 2. QC NARRATIVE(S) Subject: General Comments: I arrived at the Sharpe Site at 0800. Nature Crete crew arrived at 0900. I went to Hertz to rent an auger and a concrete saw. It was decided that we would cut additional AC necessary to place the bollards around the concrete pad. Final preparations for concrete placement were made.and Bollard posts were purchased. Don Valentine arrived on site at approximately 1030 hrs. Another visual inspection of the forms was made. Everything looked ready. A brief discussion was conducted between myself and Don about the AST placement. I informed Don of all preparatory issues for the AST placement. The concrete arrived at 1200. Two cylinders were prepared for 4 -day and 7 -day break tests. The concrete truck left at approximately 1220 hrs. We were approximately 1 cy short. A second concrete truck arrived at approximately 1345 and concrete placement was completed. The bollard placement was also completed. Final touches were made on the pad and channel. Don witnessed the concrete placement. It was decided not to make any kind of construction joints in the pad because this would defeat the definition of a containment pad. The concrete test cylinders were left in place and will be taken to Kleinfelder tomorrow. Work area was secured prior to leaving the site. Subject: Safety Inspections/Meetings: All of the safety issues discussed on Friday were reviewed today. Issues tied in to working with and around concrete. 4. PREP/INITIAL INSPECTIONS Preparatory Inspection was held today for AST RETROFITTING. Initial Inspection was held today for PLACEMENT OF CONCRETE PAD. 8.
