P Item # Remarks 112 HSC 25284.2 Failed to test spill containers annually. Annual spill container testing was last performed on 1-29-2016 and was not tested as required at the time of inspection. As such, this testing has not been completed and is past due. This test is required once every 12 months. Immediately schedule this test and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. This is a Class II violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The 7-20-2016 designated operator (DO) report listed a maintenance record for the L14 vent box alarm, however, no maintenance record was available for review at the time of inspection. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies of missing records to the EHD. This is a Class II violation.
