Some of this information has not been <br />received resulting in a non-compliant status for this facility. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is <br />billed at the current hourly rate ($125). Failure to submit the completed information immediately <br />may result in a re -inspection, additional violations, and/or further legal action. <br />The missing information includes: <br />1. Complete UST monitoring plan including model name and number of the vacuum/liquid sensors <br />in the tank annular space and piping annular space. <br />2. Complete UST monitoring site plan including the location and monitoring zone of all L and S liquid <br />and vacuum sensors. <br />3. Alarm logs and service receipts for two L alarms on April 24, 2012. <br />4. Return to Compliance Certification form, completely filled out. <br />5.
